摘要
目的 总结肝移植术后耶氏肺孢子菌肺炎的成功诊治经验,探讨肝移植术后并发肺孢子菌肺炎的预防和治疗.方法 回顾性分析中国科学技术大学附属第一医院(安徽省立医院)器官移植中心,2018年1月~2021年12月肝移植术后诊断为耶氏肺孢子菌肺炎的4例患者的临床资料,从临床症状、实验室检查、影像学表现、病情转归等方面,分析危险因素并总结成功诊治经验.结果 4例肝移植术后并发耶氏肺孢子菌肺炎患者,2例经本中心治疗好转,1例经本中心治疗后转为重症肺炎转ICU治疗后好转,1例转ICU治疗后死亡.结论 耶氏肺孢子菌肺炎是肝移植术后少见、严重的机会性感染,临床应合理预防,早识别、早诊断、早治疗,提高患者生存率.
Objective To summarize the experience of diagnosis and treatment of Pneumocystis jiroveci pneumonia(PJP)af-ter liver transplantation(LT),and to explore the prevention and treatment of PJP after LT.Methods The clinical data of 4 patients diagnosed with Pneumocystis jiroveci pneumonia after liver transplantation from January 2018 to December 2021 in the Organ Trans-plantation Center of the First Affiliated Hospital of University of Science and Technology of China(Anhui Provincial Hospital)were ret-rospectively analyzed in terms of clinical signs and symptoms,laboratory examinations,radiological characteristic and prognosis.The risk factors were analyzed and the experience of successful diagnosis and treatment was summarized.Results Among the 4 cases of pneumocystis jiroveci pneumonia after liver transplantation,2 cases were improved after treatment in our center,1 case turned to se-vere pneumonia after treatment in our center and turned to ICU,and 1 case died after being transferred to ICU.Conclusion Pneumo-cystis jiroveci pneumonia is a rare and serious opportunistic infection after liver transplantation.Rational prevention,early recognition,early diagnosis and early treatment are important to improve the survival rate of patients.
