摘要
目的:分析某三级甲等综合性医院吸入剂的使用合理性,促进吸入剂的临床合理应用。方法:统计2017年1月~2020年12月该院使用的吸入剂的消耗量、用药频度(DDDs)、限定日费用(DDC)及排序比(B/A)等指标进行分析,随机抽取呼吸与危重症科使用吸入剂的480例患者进行合理性评价。结果:2017~2019年度吸入剂消费金额呈现逐步增长趋势,2020年度受新冠疫情的影响,销售金额有所下降。2017~2020年吸入剂的DDDs排名相对稳定,其中吸入用布地奈德混悬液、吸入用复方异丙托溴铵溶液排名始终位于前2位。DDC最高的为吸入用布地奈德混悬液,约22元,而其余药物DDC均在10元以内。沙美特罗替卡松粉吸剂、噻托溴铵粉吸入剂的B/A均<1。抽查的480份医嘱点评不合理率为44.17%,主要为联合用药不适宜(30.83%)。结论:2017~2020年,该院吸入剂使用相对合理,其金额及构成比较稳定。
Objective:To analyze the reasonableness of inhalation products utilization in the hospital and promote the clinical rational application of inhalation products.Methods:The consumption,defined daily dose system(DDDs),defined daily cost(DDC)and ranking ratio(B/A)of inhalation products used in the hospital from January 2017 to December 2020 were analyzed,and 480 patients received inhalation products in the respiratory and ICU department were randomly selected for rationality evaluation.Results:From 2017 to 2019,the consumption amount of inhalation products gradual grewed.In 2020,affected by COVID-19 pandemic,the sales amount decreased.From 2017 to 2020,the ranking of DDDs of inhalation products was relatively stable,among which budesonide suspension for inhalation and compound ipratropium bromide solution for inhalation were always listed in the top 2.The highest DDC was budesonide suspension for inhalation,about 22 yuan,while the DDC of other drugs were within 10 yuan.The B/A of salmeterol ticason powder inhalation and tiotropium bromide powder inhalation were<1.The irrational rate of 480 medical instructions was 44.17%.The main reason was that the combination of drugs is inappropriate(30.83%).Conclusion:From 2017 to 2020,the use of inhalation products in the hospital was relatively reasonable,and its amount and composition were relatively stable.
