Hhcy对急性心肌梗死患者IL-6/STAT3信号通路与炎症因子水平的影响 被引量:4

Effect of Hhcy on IL-6/STAT3 signaling pathway and inflammatory factor levels in patients with acute myocardial infarction

摘要 目的探讨高同型半胱氨酸血症(Hhcy)对急性心肌梗死患者白细胞介素-6/信号转导与转录激活因子3(IL-6/STAT3)信号通路与炎症因子水平的影响。方法回顾性分析2020年1月至2022年3月在烟台业达医院住院治疗的80例急性心肌梗死患者临床资料,根据患者是否并发Hhcy,将其分为Hhcy组(n=35)和非Hhcy组(n=45)。比较两组患者基线资料差异,检测并比较两组患者炎症因子包括C反应蛋白(CRP)、肿瘤坏死因子-α(TFN-α)水平,观察IL-6/STAT3信号通路表达差异,采用Pearson相关性检验分析IL-6/STAT3、CRP、IL-6、TFN-α与急性心肌梗死并发Hhcy的关系,受试者工作特征(ROC)曲线评估其对急性心肌梗死患者并发Hhcy的预测价值。结果Hhcy组患者有高血压史、糖尿病史、高血脂史及吸烟史占比均高于非Hhcy组,差异均有统计学意义(P<0.05)。Hhcy组患者血清CRP、TFN-α、IL-6水平及STAT3表达为(18.67±3.69)mg/L、(45.69±8.64)pg/mL、(22.64±5.61)pg/mL、1.69±0.32,均明显高于非Hhcy组[(12.57±2.51)mg/L、(35.97±8.67)pg/mL、(16.52±4.72)pg/mL、1.21±0.28],差异均有统计学意义(P<0.05)。经过ROC分析证实,血清CRP、TFN-α、IL-6及STAT3可用于急性心肌梗死合并Hhcy的预测,曲线下面积分别为0.883、0.802、0.866、0.831,预测价值好(P<0.05)。血清CRP、TFN-α、IL-6水平及STAT3表达与急性心肌梗死患者Hcy水平呈正相关(r=0.531、0.529、0.527、0.632,P<0.05)。结论Hhcy与急性心肌梗死患者体内的IL-6/STAT3信号通路表达与炎症因子水平变化关系密切,当急性心肌梗死患者合并Hhcy时,会导致IL-6/STAT3信号通路表达上调,体内炎症因子水平升高。 Objective To investigate the effect of hyperhomocysteinemia(Hhcy)on interleukin-6/signal transducer and activator of transcription 3(IL-6/STAT3)signal pathway and inflammatory factor levels in patients with acute myocardial infarction.Methods The clinical data of 80 patients with acute myocardial infarction hospitalized in Yantai Yeda Hospital from January 2020 to March 2022 were retrospectively analyzed.According to whether the patients were complicated with Hhcy,they were divided into Hhcy group(n=35)and non Hhcy group(n=45).The difference of baseline data between the two groups was compared,the inflammatory factors including C-reactive protein(CRP),tumor necrosis factor-α(TFN-α)in the two groups were detected and compared,the expression difference of IL-6/STAT3 signal pathway was observed,and the relationship of IL-6/STAT3,CRP,IL-6,TFN-αand acute myocardial infarction complicated with Hhcy were analyzed by Pearson correlation test,the predictive value of the above indicators for patients with acute myocardial infarction complicated with Hhcy was evaluated by receiver operating characteristic(ROC)curve.Results The proportion of patients with hypertension,diabetes,hyperlipidemia and smoking history in the Hhcy group was higher than that in the non Hhcy group,and the differences were statistically significant(P<0.05).The levels of serum CRP,TFN-α,IL-6 and STAT3 expression of patients in Hhcy group were(18.67±3.69)mg/L,(45.69±8.64)pg/mL,(22.64±5.61)pg/mL,1.69±0.32,which were significantly higher than those in the non Hhcy group[(12.57±2.51)mg/L,(35.97±8.67)pg/mL,(16.52±4.72)pg/mL,1.21±0.28],and the differences were statistically significant(P<0.05).Serum CRP and TFN-α,IL-6 and STAT3 were confirmed by ROC analysis can be used to predict acute myocardial infarction complicated with Hhcy,and the areas under the curve were 0.883,0.802,0.866 and 0.831 respectively(P<0.05).The Pearson test showed that the level of serum CRP,TFN-α,IL-6 and STAT3 were positively correlated with the level of Hcy in patients with acute myocardial infarction(r=0.531,0.529,0.527,0.632,P<0.05).Conclusion The expression of IL-6/STAT3 signal pathway in patients with acute myocardial infarction is closely related to the level of inflammatory factors.When patients with acute myocardial infarction are combined with Hhcy,the expression of IL-6/STAT3 signal pathway will be up-regulated and the level of inflammatory factors in vivo will be increased.
