摘要
目的应用二维斑点追踪(2D-STI)技术评估痛风患者左室收缩功能。方法选取60例痛风患者,根据病程分为痛风Ⅰ组(病程<10年,30例)和痛风Ⅱ组(病程≥10年,30例),另选同期健康成人30例为对照组。应用经胸超声心动图测量室间隔厚度(IVS)、左室舒张末期内径(LVEDD)、左室后壁厚度(LVPW)、左室收缩末期内径(LVESD)、左室舒张末期容积(LVEDV)、左室收缩末期容积(LVESV)和左室射血分数(LVEF);应用2D-STI技术获取左室整体纵向应变(LVGLS)和心尖四腔心、两腔心、三腔心切面左室纵向应变(LVLS-AP4、LVLS-AP2、LVLS-AP3);比较各组上述参数的差异。分析LVGLS与血清尿酸、肌酐水平及病程的相关性。结果痛风Ⅰ、Ⅱ组血清尿酸及肌酐水平均高于对照组,差异均有统计学意义(均P<0.05)。痛风Ⅰ、Ⅱ组IVS、LVEDD、LVPW、LVESD、LVM及LVMI与对照组比较差异均有统计学意义(均P<0.05),但痛风Ⅰ组与痛风Ⅱ组比较差异均无统计学意义;痛风Ⅰ、Ⅱ组LVGLS、LVLS-AP4、LVLS-AP2、LVLS-AP3均小于对照组,痛风Ⅰ组与痛风Ⅱ组比较差异均有统计学意义(均P<0.05)。相关性分析显示,痛风患者LVGLS与病程呈负相关(r=-0.477,P<0.05),与血清尿酸、肌酐水平均无相关性。结论应用2D-STI技术可评估痛风患者左室收缩功能受损情况,随着病程延长,其左室收缩功能受损程度加重。
Objective tracking imaging(2D-STI)technology.MethodsA total of 60 gout patients were selected and divided into groupⅠ(course of disease<10 years,30 cases)and groupⅡ(course of disease≥10 years,30 cases)according to the course of disease,and 30cases of healthy subjects(control group)were selected.The ventricular septal thickness(IVS),left ventricular end diastolic diameter(LVEDD),left ventricular posterior wall thickness(LVPW),left ventricular end systolic diameter(LVESD),left ventricular end diastolic volume(LVEDV),left ventricular end systolic volume(LVESV)and left ventricular ejection fraction(LVEF)were measured by transthoracic echocardiography,while the left ventricular global longitudinal strain value(LVGLS)and the apical four chamber,two chamber and three chamber longitudinal strain(LVLS-AP4,LVLS-AP2,LVLS-AP3)were obtained by 2D-STI technology,and the differences of the above parameters among groups were compared.The correlation between LVGLS and serum uric acid,creatinine and course of disease was analyzed.ResultsThe levels of serum uric acid and creatinine in gout groupⅠand groupⅡwere significantly higher than those in the control group(all P<0.05).There were significant differences in IVS,LVEDD,LVPW,LVESD,LVM and LVMI between gout groupⅠand groupⅡand the control group(all P<0.05),but there were no significant difference in these parameters between gout groupⅠand groupⅡ.The LVGLS,LVLS-AP4,LVLS-AP2,LVLS-AP3 in gout groupⅠand groupⅡwere lower than those in the control group(all P<0.05),and there were significant difference in these parameters between gout groupⅠand groupⅡ(all P<0.05).Correlation analysis showed that LVGLS in gout patients was not correlated with serum uric acid and creatinine levels,but had a negative correlation with the course of the disease(r=-0.477,P<0.05).Conclusion2D-STI technology can evaluate the damage of left ventricular systolic function in gout patients,and the degree of left ventricular systolic function damage increased with the prolongation of the disease.
