乳酸、肌红蛋白及心肌肌钙蛋白I水平与新生儿窒息伴心肌损伤的关系 被引量:2

Relationship between Lactate,Myoglobin and Cardiac Troponin I Levels and Neonatal Asphyxia with Myocardial Injury

摘要 目的研究乳酸(LA)、肌红蛋白(Mb)、心肌肌钙蛋白I(cTn I)与新生儿窒息伴心肌损伤的关系。方法对2014年6月—2020年6月收治92例新生儿窒息患儿的临床资料进行回顾性分析,统计新生儿窒息伴心肌损伤发生情况,对比不同心肌损伤程度患儿血清LA、Mb及cTn I水平,分析影响新生儿窒息伴心肌损伤的独立危险因素。结果本研究92例中未发生心肌损伤(未损伤组)56例,占比60.87%;发生心肌损伤(损伤组)36例,占比39.13%。损伤组血清LA、Mb及cTn I水平高于未损伤组(P<0.01)。重度期患儿血清LA、Mb及cTn I水平高于轻度期患儿(P<0.01)。经多因素Logistic回归分析发现,母体代谢性疾病、新生儿感染、子宫痉挛或出血、脐带扭转及患儿血清LA、Mb、cTn I为新生儿窒息伴心肌损伤的独立危险因素(P<0.01)。结论血清LA、Mb、cTn I可作为新生儿窒息伴心肌损伤的重要诊断指标,有利于患儿心肌损伤的早期诊断,提高围产期新生儿的存活质量。 Objective To study the relationship between lactic acid(LA),myoglobin(Mb),cardiac troponin I(cTnI)and neonatal asphyxia with myocardial injury.Methods The clinical data of 92 children with neonatal asphyxia treated from June 2014 to June 2020 were retrospectively analyzed.The incidence of neonatal asphyxia with myocardial injury was statistically analyzed,and the serum levels of LA,Mb and cTnI in children with different degrees of myocardial injury were compared.The independent risk factors of neonatal asphyxia with myocardial injury were analyzed.Results Of 92 cases in this study,there was no myocardial injury in 56(uninjury group),accounting for 60.87%.Myocardial injury occurred in 36 cases(injury group),accounting for 39.13%.The serum levels of LA,Mb and cTnI in injury group were higher than those in uninjury group(P<0.01).The serum levels of LA,Mb and cTnI in patients in severe stage were higher than those in mild stage(P<0.01).Multiple Logistic regression analysis showed that maternal metabolic disease,neonatal infection,uterine spasm or bleeding,umbilical cord torsion and serum LA,Mb,cTnI of children were independent risk factors of neonatal asphyxia with myocardial injury(P<0.01).Conclusion Serum LA,Mb and cTnI can be used as important diagnostic indicators of neonatal asphyxia accompanied by myocardial injury,which is beneficial to the early diagnosis of myocardial injury in children and improvement of the quality of life of perinatal newborns.
