盆底肌电刺激联合盆底肌训练治疗自然分娩后压力性尿失禁的临床运用探讨 被引量:3

摘要 目的 探讨自然分娩后压力性尿失禁产妇应用盆底肌电刺激联合盆底肌训练的效果。方法 方便选取2020年2月—2021年10月于福建省莆田市第一医院进行治疗的78例自然分娩后压力性尿失禁产妇,随机分成两组,各39例,对照组产妇进行盆底肌训练,观察组产妇联合采用盆底肌训练及盆底肌电刺激治疗,对比两组治疗前后产妇盆底肌表面电信号、治疗后两组盆底肌力、治疗前后产妇漏尿量及尿失禁生活质量问卷(I-QOL)评分。结果 治疗后,观察组与对照组产妇Ⅰ类肌纤维电位及Ⅱ类肌纤维电位均升高,差异有统计学意义(P<0.05),且观察组Ⅰ类肌纤维电位及Ⅱ类肌纤维电位均更高,差异有统计学意义(t=9.042、12.307,P<0.001)。观察组盆底肌正常率(92.31%)明显高于对照组(71.79%),差异有统计学意义(χ^(2)=5.571,P=0.018)。治疗后,观察组与对照组产妇漏尿量均减少,观察组漏尿量更少且I-QOL评分更高,差异有统计学意义(t=99.656、5.701,P<0.001)。结论 自然分娩后压力性尿失禁产妇应用盆底肌电刺激联合盆底肌训练能够使其尿失禁症状及盆底肌力得到有效改善,值得临床推荐应用。 Objective To explore the effect of pelvic floor muscle electrical stimulation combined with pelvic floor muscle training for women with stress urinary incontinence after natural delivery. Methods Convenient selection 78women with stress urinary incontinence after natural delivery who were treated in the First Hospital of Putian City, Fujian Province from February 2020 to October 2021 were randomly divided into two groups, 39 cases in each group. The control group was given pelvic floor muscle training, while the observation group was given pelvic floor muscle training combined with pelvic floor muscle electrical stimulation. The electrical signals of pelvic floor muscle surface, pelvic floor muscle strength, urinary leakage and urinary incontinence quality of life questionnaire(I-QOL) scores before and after treatment were compared between the two groups. Results After treatment, both the class Ⅰ muscle fiber potential and class Ⅱ muscle fiber potential of the puerperae in the observation group and the control group increased, and the difference was statistically significant(P<0.05), the class Ⅰ muscle fiber potential and class Ⅱ muscle fiber potential in the observation group were higher, the difference was statistically significant(t=9.042, 12.307, P<0.001).The normal rate of pelvic floor muscles in the observation group(92.31%) was significantly higher than that in the control group(71.79%), the difference was statistically significant(χ^(2)=5.571, P=0.018). After treatment, the amount of urine leakage of puerperae in the observation group and the control group were reduced, the observation group had less urine leakage and higher I-QOL score, and the difference was statistically significant(t=99.656, 5.701, P<0.001).Conclusion The application of pelvic floor muscle electrical stimulation combined with pelvic floor muscle training can effectively improve the symptoms of urinary incontinence and pelvic floor muscle strength of women with stress urinary incontinence after natural delivery, which is worthy of clinical recommendation.
