摘要
目的 了解类鼻疽肺病与其它特殊病原体所致肺病的差异,有助于肺病的鉴别。方法 采用回顾性研究,分析海南地区380例肺病患者的流行病学、基础疾病、临床特征、实验室指标、胸部影像学特征差异。结果 不同性别间类鼻疽伯克霍尔德菌(Burkholderia pseudomallei, BP)组与肺曲霉菌(Pulmonary aspergillus, PA)组、肺隐球菌(Pulmonary cryptococcus, PC)组、肺诺卡菌(Pulmonary nocardia, PN)组、马尔尼菲青霉菌(Penincillium Marneffei, PM)组、非结核分枝杆菌(Nontuberculous mycobacteria, NTM)组、白色念珠菌(Candida albicans, CA)组、立克次体(Rickettsial disease, RD)组有显著差异(P<0.05),年龄间BP组与PA组、PN组、NTM组、CA组有显著差异(P<0.05)。2型糖尿病间BP组与PA组、PC组、PN组、PM组、NTM组、CA组、RD组有显著差异(P<0.05)。白细胞计数和中性粒细胞比率间BP组与PC组、NTM组、RD组有显著差异(P<0.05),超敏C-反应蛋白、降钙素原、红细胞沉降率间BP组与PA组、PC组、PN组、NTM组、CA组、RD组有显著差异(P<0.05)。结论 类鼻疽肺病与其它特殊病原体所致肺病的差异主要是性别、2型糖尿病、超敏C-反应蛋白、降钙素原、红细胞沉降率。
Objective To understand the differences between melioidosis pneumonia and other specific pathogenic pneumopathies, and to help the identification of lung disease. Methods A retrospective study was conducted on the differences in epidemiology, underlying diseases, clinical features, laboratory indicators, and chest imaging features of 380 patients with lung disease in the Hainan area.Results There were significant differences in genders between the Burkholderia pseudomallei(BP) group and Pulmonary aspergillus(PA) group, Pulmonary cryptococcus(PC) group, Pulmonary nocardia(PN) group, Penicillium Marneffei(PM) group, Nontuberculous mycobacteria(NTM) group, Candida albicans(CA) group, and Rickettsial disease(RD) group(P<0.05).There were significant differences in age between the BP group and PA group, PN group, NTM group, and CA group(P<0.05). There were significant differences between the BP group and PA group, PC group, PN group, PM group, NTM group, CA group, and RD group in type 2 diabetes mellitus(P<0.05). There were significant differences in WBC count and neutrophil ratio between the BP group and PC group, NTM group, and RD group(P<0.05), and there were significant differences in hypersensitive C-reactive protein, procalcitonin and erythrocyte sedimentation rate between BP group and PA group, PC group, PN group, NTM group, CA group, and RD group(P<0.05).Conclusion The differences between melioidosis pneumonia and other special pathogen lung diseases are mainly gender, type 2 diabetes, hypersensitive C-reactive protein, procalcitonin, and erythrocyte sedimentation rate.
