经血管外膜组织液传输对心肌灌注的研究 被引量:1

摘要 目的比较经由血管外膜组织液传输或血液循环的示踪剂在大鼠心肌组织中的分布差异,明确经血管外膜组织液传输的心肌灌注特点。方法采用组织学和Micro-CT成像方法,分别研究荧光素钠、硝酸银经由下肢血管外膜传输在心肌组织中灌注的特点。本研究选取2021年9月至2022年9月由中国医学科学院实验动物研究所提供的25只SD大鼠,随机分为空白对照组、血液灌注10 min组、外膜传输1 min组、5 min组和10 min组共5组,每组各5只。分别在右下肢血管外膜附近、或右下肢静脉管腔内给予生理盐水、硝酸银溶液或荧光素钠,留取下肢血管及其周围组织、心肺组织标本,分别进行组织学和Micro-CT成像研究,分别比较荧光素钠和银信号的分布差异。结果组织学分析显示血管外膜给予的荧光素钠并非进入血液,而是经由下肢血管外膜组织液向心性传输进入心肌组织。Micro-CT结果发现,经由下肢血管外膜组织液传输后,在近心端的血管外膜及其周围组织中有大量银信号,而远心端很少[(13.9±0.6)×10^(3)μm^(2)比(0.0±0.0)×10^(3)μm^(2),P<0.0001];血液灌注组下肢血管的近心端和远心端的银信号分布则无显著性差异[(16.2±0.8)×10^(4)μm^(2)比(14.3±0.5)×10^(4)μm^(2),P=0.1976]。分别比较不同时间段的结果,发现随着时间的增加,经由血管外膜传输的示踪剂逐渐分布在心肌组织中,而在心腔内部的分布很少。血液灌注组心肺组织的银信号主要分布于心腔内,而心肌组织中量少。结论与经血液循环的心肌灌注相比,经由下肢血管外膜组织液传输的示踪剂在心肌组织中分布的量更多。说明心肌组织灌注很可能存在血液灌注和血管外膜传输两种途径。 Objective To explore the characteristics of myocardial perfusion transmitted by vascular adventitial interstitial fluid(ISF).Methods Histological and Micro-CT imaging methods were used to detect the myocardial perfusion characteristics of fluorescein sodium or silver nitrate transmitted by adventitial ISF along blood vessels of lower extremities.A total of 25 SD rats were included in this study at the Institute of Laboratory Animal Sciences,Chinese Academy of Medical Sciences from September 2021 to September 2022.The SD rats were randomly divided into the control group,hemoperfusion-10 min group,adventitial transport-1 min group,adventitial transport-5 min group and adventitial transport-10 min group,with 5 rats in each group.Normal saline,silver nitrate,or fluorescein sodium was administered near the vascular adventitia or in the venous cavity of the right lower limb;and samples of the blood vessels and surrounding tissues of the lower limb,as well as cardiopulmonary tissues were collected for histological and Micro-CT imaging studies,respectively,to compare the differences in the distribution of fluorescein sodium and silver signals.Results Histological analysis showed that the fluorescein sodium given in the vascular adventitia did not enter the blood but entered the myocardium centripetally through the transport of the adventitial ISF along blood vessels of lower extremities.Micro-CT results showed that,after transported by the lower extremities vascular adventitial ISF,there were a large amount of silver signals in the proximal vascular adventitia and surrounding tissues,but little in the distal[(13.9±0.6)×10^(3)μm^(2)vs.(0.0±0.0)×10^(3)μm^(2),P<0.0001];there was no significant difference in the distribution of silver signals between the proximal and distal ends of lower limb vessels in the hemoperfusion group[(16.2±0.8)×10^(4)μm^(2)vs.(14.3±0.5)×10^(4)μm^(2),P=0.1976].Comparing the results of different time periods,it was found that with the increase of time,the tracer transported through the vascular adventitia gradually distributed in the myocardium,while the distribution in the heart cavity was very small.The silver signals of cardiopulmonary tissues in the blood perfusion group were mainly distributed in the heart cavity,while the amount of silver signals in myocardial tissues was small.Conclusion Tracers transported via adventitial ISF of lower extremity vessels are distributed in greater amounts in myocardial tissue compared with myocardial perfusion via blood circulation.It shows that myocardial perfusion probably has two ways:blood perfusion and adventitial ISF transport.
