摘要
目的:探讨氯普鲁卡因复合舒芬太尼用于硬膜外分娩镇痛的效果。方法:选择2019年7月至2021年5月在我院实施硬膜外分娩镇痛初产妇60例,年龄24~40岁,BMI20~28kg/m 2,ASA分级Ⅰ级,随机分为L组和R组,每组30例。两组产妇均实施硬膜外分娩镇痛,L组氯普鲁卡因600mg+舒芬太尼50μg+生理盐水稀释至100mL,R组罗哌卡因100mg+舒芬太尼50μg+生理盐水稀释至100mL。记录两组产妇镇痛前、镇痛后5min、镇痛后30min、胎儿娩出时疼痛VAS评分、改良Bromage评分、胎心率;记录两组产妇第一产程、第二产程时间;记录两组胎儿娩出后1min和5min的Apgar评分。结果:两组产妇镇痛后5min、镇痛后30min、胎儿娩出后的疼痛VAS评分较镇痛前明显降低(P<0.05);L组镇痛后5min疼痛VAS评分较R组明显降低(P<0.05),其余时间点两组产妇疼痛VAS评分比较无明显差异(P>0.05);两组产妇第一产程和第二产程比较无明显差异(P>0.05);两组胎儿娩出后1min和5min的Apgar评分比较无明显差异(P>0.05)。结论:氯普鲁卡因复合舒芬太尼用于硬膜外分娩镇痛,效果与罗哌卡因复合舒芬太尼相当,但氯普鲁卡因起效更快,不影响产程和新生儿评分。
Objective:To investigate the analgesic effect of chloroprocaine combined with sufentanil in epidural labor.Methods:From July 2019 to May 2021,60 primipara,aged 24~40years,with BMI of 20~28kg/m 2 and ASA grade I,who were performed epidural labor analgesia in our hospital,were randomly divided into L group and R group,with 30 cases in each group.Both groups of parturients underwent epidural labor analgesia.In group L,chloroprocaine 600mg+sufentanil 50μg+normal saline was diluted to 100mL,and in group R,ropivacaine 100mg+sufentanil 50μg+normal saline was diluted to 100mL.The VAS score,modified bromage score and fetal heart rate were recorded before analgesia,5 min after analgesia,30 min after analgesia,and at the time of fetal delivery in both groups.The first and second stages of labor were recorded.Apgar scores were recorded 1 min and 5 min after fetal delivery in both groups.Results:The VAS scores of pain 5 minutes after analgesia,30 minutes after analgesia,and after fetal delivery in both groups were significantly lower than those before analgesia(P<0.05).The VAS score of pain in group L was significantly lower than that in group R at the 5 minutes after analgesia(P<0.05),and there was no significant difference between the two groups at other time points(P>0.05).There was no significant difference between the two groups in the first and second stages of labor(P>0.05).There was no significant difference in Apgar scores between the two groups at 1 min and 5 min after delivery(P>0.05).Conclusion:The analgesic effect of chloroprocaine combined with sufentanil in epidural labor is comparable to that of ropivacaine combined with sufentanil,but chloroprocaine has a faster onset,and does not affect the labor process and neonatal score.
