摘要
目的通过实施PDSA循环法,提高出院病案3日归档率、7日归档率。方法描述某医院2018年7月—2019年12月出院病案3日归档率、7日归档率情况,运用卡方检验对比分析PDSA循环法实施前(2018年7月—2019年3月)和实施后(2019年4月—2019年12月)出院病案3日归档率、7日归档率的变化情况,验证PDSA循环法在提高住院病案归档率管理中的实施效果。结果出院病案3日归档率从74.9%提升到88.6%,7日归档率从95.0%提升到98.4%,实施PDSA循环法前后比较,差异具有统计学意义。在PDSA循环法实施4个月后,出院病案3日归档率均在90.0%以上,7日归档率均在99.0%以上。PDSA方法实施后,各临床科室的3日归档率、7日归档率差异均有统计学意义(P<0.05)。结论运用PDSA循环法管理病案归档,使得病案3日归档率、7日归档率得到显著提高。同时提高了医护人员对病案及时归档重要性的认识,提升了病案管理部门工作效率,改善了服务质量。
Objective To Improve archiving rate of the medical records of patients discharged from hospitals in three days and in seven days by applying PDSA(Plan-Do-Study-Act)cycle.Methods The three-day and seven-day archiving rate of the medical records of patients discharged form hospitals from July 2018 to December 2019 in a hospital was recorded.Chi-square test was used for comparisons.The change of three-day and seven-day archiving rate of medical record was collected in two periods:from July 2018 to March 2019 and from April 2019 to December 2019.Results The three-day archiving rate of medical record was increased from 74.9%to 88.6%and the seven-day archiving rate of medical record was increased from 95.0%to 98.4%.After four months of applying PDSA cycle,the three-day archiving rate of the medical records was increased above 90.0%,and the seven-day archiving rate of the medical records increased above 99.0%.The three-day and seven-day archiving rate of the medical records of clinical departments were statistically different(P<0.05).Conclusion The application of PDSA cycle to the management of medical records has significantly improved the three-day and seven-day archiving rates of medical records.Meanwhile,it improves the awareness of medical staff on the importance of filing medical records in time,enhances the work efficiency of management department of medical records and elevates the service quality.
