摘要
目的 探讨急性缺血性脑卒中(acute ischemic stroke, AIS)患者血清脂蛋白相关磷脂酶(lipoproteinassociated phospholipase A2, Lp-PLA2)水平与病情严重程度及预后的相关性。方法 选择2020年1月—2021年11月就诊于南京市江宁医院的96例AIS患者为研究组,选取96名同期体检的健康体检者为对照组。抽取两组静脉血测定血清Lp-PLA2水平。对比两组受检者血清Lp-PLA2水平差异,对比不同病情严重程度及预后AIS患者血清Lp-PLA2水平,并分析血清Lp-PLA2水平与AIS患者病情严重程度及预后的相关性。结果研究组血清Lp-PLA2水平为(427.26±144.16)μg/L,高于对照组的(188.96±52.65)μg/L,差异有统计学意义(t=15.213,P<0.05)。针对不同病情严重程度患者血清Lp-PLA2水平,随着病情加重Lp-PLA2水平逐渐升高,差异有统计学意义(P<0.05)。预后不良组血清Lp-PLA2水平为(591.21±146.80)μg/L,高于预后良好组的(347.31±95.21)μg/L,差异有统计学意义(t=9.782,P<0.05)。Pearson相关性分析显示:血清Lp-PLA2水平与AIS患者病情严重程度呈正相关(r=0.570,P<0.001),与其预后呈负相关(r=-0.547,P<0.001)。结论 AIS患者血清Lp-PLA2水平异常升高,且其水平高低与患者病情严重程度和预后密切相关。
Objective To investigate the correlation between serum lipoprotein-associated phospholipase A2(LpPLA2) levels and disease severity and prognosis in patients with acute ischemic stroke(AIS). Methods 96 AIS patients who visited Nanjing Jiangning Hospital from January 2020 to November 2021 were selected as the study group,and 96 health examinees who underwent physical examination at the same time were selected as the control group.The serum Lp-PLA2 level was measured by venous blood from two groups. The difference of serum Lp-PLA2 level between the two groups was compared, the serum Lp PLA2 level of AIS patients with different severity and prognosis was compared, and the correlation between serum Lp-PLA2 level and the severity and prognosis of AIS patients was analyzed. Results The serum Lp-PLA2 level in the study group was(427.26±144.16) μg/L, higher than that of the control group(188.96±52.65) μg/L, the difference was statistically significant(t=15.213, P<0.05). The serum Lp-PLA2 level in patients with different disease severity increased gradually with the aggravation of the disease, and the difference was statistically significant(P<0.05). The serum Lp-PLA2 level in the poor prognosis group was(591.21±146.80) μg/L,higher than that of the group with good prognosis(347.31±95.21) μg/L, the difference was statistically significant(t=9.782, P<0.05). Pearson correlation analysis shows that serum Lp-PLA2 level was positively correlated with the severity of AIS(r=0.570, P<0.001), and negatively correlated with the prognosis(r=-0.547, P<0.001). Conclusion Serum Lp-PLA2 levels were abnormally elevated in patients with AIS, and the level was closely correlated with the severity of patients’ disease and prognosis.
