骨性Ⅱ类错[牙合]畸形患儿正畸治疗前后上气道矢状径的变化及影响因素logistic归因分析 被引量:5

摘要 目的:分析影响骨性Ⅱ类错[牙合]畸形患儿正畸治疗前后上气道矢状径变化的影响因素。方法:选取我院2019年4月~2022年4月收治的骨性Ⅱ类错[牙合]畸形患儿128例,依据随机数表法将其分为对照组、观察组,各64例。观察组采用Twin-block矫治器进行正畸治疗,对照组采用固定矫治器正畸治疗。矫治器前后拍摄头颅侧位片,用WinCeph8.0软件测量相关数据。比较两组患儿治疗前后上气道矢状径(鼻咽、腭咽、舌咽、喉咽)相关指标,根据上气道矢状径指标变化情况将患儿分为显著变化组、无显著变化组,采用二元logistic回归分析影响治疗后上气道矢状径变化的因素。结果:矫治后观察组SNA°、SNB°、ANB°较矫治前减小,且SNA°、ANB°小于对照组(P<0.05)。与矫治前相比,矫治后观察组PNS-Ba、PNS-UPW、SPP-SPPW、U-MPW、Mc1-Mc2、PAS、V-LPW均有升高,且PNS-UPW、SPP-SPPW、U-MPW、Mc1-Mc2、PAS、V-LPW高于对照组(P<0.05)。128例患儿接受正畸治疗后52例患儿上气道矢状径有显著变化,显著变化概率为40.63%。二元logistic回归分析显示正畸治疗方式、颈椎骨龄定量分期、矫治前ANB°是影响正畸治疗后上气道矢状径显著变化的因素(P<0.05)。结论:Twin-block矫治器可增加骨性Ⅱ类错[牙合]畸形患儿上气道矢状径,正畸治疗方式、颈椎骨龄定量分期、矫治前ANB°是正畸治疗后上气道矢状径显著变化的影响因素。 Objective:To analyze the influencing factors of sagittal diameter of upper airway in children with skeletal Class II malocclusion before and after orthodontic treatment.Methods:Totally 128 children with skeletal Class II malocclusion received from April 2019 to April 2022 in our hospital were sampled and divided into control group and observation group according to random number table method,with 64 cases in each of the two groups.The observation group was treated with Twin-block orthodontic appliance,and the control group was treated with fixed orthodontic appliance.All children were taken lateral cephalic X-ray films before and after the appliance,and relevant data were measured with WinCeph 8.0 software.To compare the relevant indexes of the upper airway sagittal diameter(nasopharynx,palatopharynx,glossopharynx,laryngopharynx)of the two groups of children before and after treatment,and divide the children into significant change group and no significant change group according to the changes of the upper airway sagittal diameter indexes,and use binary logistic regression to analyze the factors affecting the changes of the upper airway sagittal diameter before and after orthodontic treatment.Results:After treatment,the SNA°,SNB°and ANB°in the observation group decreased compared with those before treatment,and the SNA°and ANB°in the observation group were smaller than those in the control group(P<0.05).Compared with before treatment,PNS-Ba,PNS-UPW,SPP-SPPW,U-MPW,Mc1-Mc2,PAS,V-LPW in the observation group after treatment were all increased,and PNS-UPW,SPP-SPPW,U-MPW,Mc1-Mc2,PAS,V-LPW in the observation group were higher than those in the control group(P<0.05).After 128 children received orthodontic treatment,52 children had significant changes in the sagittal diameter of the upper airway,with a significant change probability of 40.63%.The binary logistic regression analysis showed that the orthodontic treatment method,the quantitative staging of cervical spine bone age,and the ANB°before orthodontic treatment were the factors influencing the significant changes in the sagittal diameter of the upper airway after orthodontic treatment(P<0.05).Conclusion:Twin-block appliance can increase the sagittal diameter of upper airway in children with skeletal Class II malocclusion.Orthodontic treatment methods,quantitative staging of cervical vertebral bone age,and ANB°before orthodontic treatment are the influencing factors of significant changes in sagittal diameter of upper airway after orthodontic treatment.
