摘要
目的观察罂粟碱注射液与胰激肽原酶联合治疗对糖尿病周围神经病变患者整体获益水平的影响,以期为临床相关疾病治疗提供科学参考。方法纳入河南中医药大学第一附属医院2021年6月—2022年6月期间收治的100例糖尿病周围神经病变患者,按随机数表法分为两组,各50例,对照组给予常规治疗联合盐酸罂粟碱注射液治疗,研究组在对照组基础上联合注射用胰激肽原酶治疗,对比两组治疗效果、肌电图相关指标[分别于治疗前、治疗结束时,对比上下肢运动神经速度(MNCV)与神经传导速度(SNCV),并观察相应波形的动作电位平均波幅变化及H反射速度情况]、疼痛程度[分别于治疗前、治疗结束时,采用视觉模拟评分法(VAS)]。结果研究组治疗总有效率较对照组高,差异有统计学意义(P<0.05);治疗前,两组腓总神经及胫总神经MNCV、腓总神经及胫总神经SNCV、AP平均波幅、H反射对比,差异无统计学意义(P>0.05),治疗结束时,腓总神经及胫总神经MNCV、腓总神经及胫总神经SNCV均升高,AP平均波幅、H反射均下降(P<0.05),且与对照组相比,研究组腓总神经及胫总神经MNCV、腓总神经及胫总神经SNCV、AP平均波幅均较高,HP反射较低,差异有统计学意义(P<0.05);治疗前,两组VAS评分对比无统计学意义(P>0.05);治疗结束后,两组VAS评分均下降,且研究组VAS较对照组低,差异有统计学意义(P<0.05)。结论盐酸罂粟碱注射液联合注射用胰激肽原酶治疗糖尿病周围神经病变患者,效果显著,利于缓解患者疼痛程度,改善神经传导运动速度。
Objective To observe the efficacy of papaverine injection combined with Pancreatic kininogenase in the treatment of diabetes peripheral neuropathy,which is expected to provide scientific reference for the treatment of clinical related diseases.Methods A total of 100 patients with diabetic peripheral neuropathy admitted to our hospital from June 2021 to June 2022 were included and divided into two groups according to the random number table method,with 50 cases patients in each.The control group took routine treatment combined with papaverine hydrochloride injection treatment,and the study group was treated with pancreatic kininogenase for injection on the basis of the control group.The therapeutic effects,the electromyography related indicators[before treatment and at the end of treatment,the motor nerve conduction velocity(MNCV)and sensory nerve conduction velocity(SNCV)of the upper and lower limbs were compared,and the corresponding waveform average amplitude of action potential changes and H-reflex velocity were observed],and the degree of pain[taking visual analogue scale(VAS)before treatment and at the end of treatment]of the two groups were compared.Results The total effective rate of treatment of the study group was higher than that of the control group,with a statistically significant difference(P<0.05);Before treatment,there were no significant differences between the two groups in terms of MNCV of common peroneal nerve and common tibial nerve,SNCV of common peroneal nerve and common tibial nerve,average amplitude of AP and H-reflex(P>0.05).At the end of treatment,the common peroneal nerve and common tibial nerve the of MNCV and common peroneal nerve and common tibial nerve the of SNCV increased,and the average amplitude of AP and H-reflex decreased,with statistically significant differences(P<0.05);compared with the control group,the common peroneal nerve and common tibial nerve MNCV,common peroneal nerve and common tibial nerve SNCV and average amplitude of AP in the study group were higher,and the HP-reflex was lower,with statistically significant differences(P<0.05).Before treatment,there was no significant difference in scores of VAS between the two groups(P>0.05);After treatment,the VAS score of the two groups decreased,and the VAS of the study group was lower than that of the control group,with a statistically significant difference(P<0.05).Conclusion Papaverine hydrochloride injection combined with Pancreatic kininogenase powder for injection has a significant effect in the treatment of patients with diabetic peripheral neuropathy,which is beneficial to alleviate the degree of patients’pain and improve the nerve conduction velocity.
