摘要
糖尿病肾病是由于糖尿病引发的肾脏损害,临床表现主要以蛋白尿为主,随着病程延长,可出现肾功能异常,最终发展为终末期肾病。糖尿病肾病一旦发展为终末期肾病,治疗往往比较困难,故而应给予及时的防治以延缓糖尿病肾病的进展。目前,糖尿病肾病的临床治疗尚无特效药,主要是血管紧张素转换酶抑制剂/血管紧张素受体拮抗剂、钠葡萄糖协同转运蛋白2抑制剂、血液净化、肾移植等治疗。近年来中医药因其疗效确切、体系独特,在糖尿病肾病诊疗中发挥了重要作用。中医治疗糖尿病肾病主要有内服法和外治法,根据患者不同的临床表现辨证论治选方用药,外治法主要有针灸、药浴等。本文旨在对近年来中西医结合治疗糖尿病肾病的相关研究进行归纳整理,从分期论治、康复护理、营养干预、生活质量4个方面进行综述,以期为糖尿病肾病的治疗提供新的思路。
Diabetes kidney disease(DKD)is renal damage caused by diabetes mellitus(DM).The main clinical manifestations are proteinuria.With the prolongation of the disease course,renal dysfunction may occur and eventually develop into end-stage renal disease.Once DKD develops into end-stage renal disease,it is often more difficult to treat.Therefore,timely prevention and treatment should be given to delay the progression of DKD.At present,there are no specific drugs for the clinical treatment of DKD,mainly angiotensin converting enzyme inhibitors/angiotensin receptor antagonists,sodium glucose cotransporter 2 inhibitors,blood purification,kidney transplantation and other treatments.In recent years,traditional Chinese medicine has played an important role in the diagnosis and treatment of DKD due to its exact curative effect and unique system.Traditional Chinese medicine treatment of DKD mainly includes internal treatment and external treatment.According to the different clinical manifestations of patients,prescription drugs are selected based on syndrome differentiation,and external treatment methods mainly include acupuncture and moxibustion,medicinal bath,etc.This article aims to summarize the related research on the treatment of DKD with integrated traditional Chinese and Western medicine in recent years,and review from four aspects:treatment by stages,rehabilitation nursing,nutritional intervention and quality of life,in order to provide new ideas for the treatment of DKD.
