摘要
目的 研究表皮生长因子(EGFR)变阴突性非小细胞肺癌(NSCLC)脑转移患者的临床特征。方法 选取92例EGFR突变阴性NSCLC患者为阴性组,并以1∶1比例选取同期92例EGFR突变阳性患者作为阳性组。分析NSCLC的脑转移发病率、病灶数目、大小、发病症状。结果 阴性组3年累积脑转移率28.26%(26/92)较阳性组46.74%(43/92)低(P<0.05)。本组26例EGFR突变阴性NSCLC脑转移中,病灶部位幕上61.54%(16/26)、幕下3.85%(1/26)、幕上幕下同时累积30.77%(8/26)、侵犯颅骨3.85%(1/26),其中以顶叶和额叶较为常见。脑转移病灶数1个、2~3个、>3个分别为9例(34.62%)、9例(34.62%)、8例(30.77%)。病灶大小范围<1.0 cm、<2.0 cm、<3.0 cm分别为7例(26.92%)、9例(34.62%)、10(38.46%)。症状:呼吸系统症状46.15%(12/26)、神经系统症状26.92%(7/26)、疼痛11.54%(3/26)、胸部影像学异常11.54%(3/26)、食欲降低3.85%(1/26),其中神经系统症状中包含瘫痪15.38%(4/26)、癫痫3.85%(1/26)、头痛3.85%(1/26)、运动障碍3.85%(1/26)。结论 EGFR敏感突变阴性NSCLC患者脑转移风险较低,发病部位主要为幕上,主要表现为呼吸系统症状和神经系统症状。
Objective To study the clinical characteristics of patients with epidermal growth factor(EGFR)-negative non-small cell lung cancer(NSCLC).Methods 92 EGFR mutation-negative NSCLC patients were selected as the negative group, and 92 EGFR mutation-positive patients were selected in a 1∶1 ratio.The incidence, number, size, and symptoms of brain metastasis in NSCLC were analyzed.Results The 3-year cumulative brain metastasis rate of 28.26%(26/92) was lower than 46.74%(43/92) in the positive group(P<0.05).Among the 26 cases of EGFR mutation-negative NSCLC brain metastases, 61.54% were supratentorial(16/26),3.85%(1/26),30.77%(8/26),and 3.85%(1/26),among which the parietal and frontal lobe were common.The number of brain metastatic lesions was 1,2~3,and> 3 was 9(34.62%),9(34.62%),and 8(30.77%),respectively.The lesion size range <1.0 cm, <2.0 cm, <3.0 cm was 10(38.46%),14(53.85%),and 16(61.54%),respectively.Symptoms: 4 Respiratory symptoms 46.15%(12/26),neurological symptoms 26.92%(7/26),pain 11.54%(3/26),chest imaging abnormalities 11.54%(3/26),appetite loss 3.85%(1/26),and neurological symptoms include paralysis 15.38%(4/26),epilepsy 3.85%(1/26),headache 3.85%(1/26),motor impairment 3.85%(1/26).Conclusion EGFR sensitive mutation-negative NSCLC patients have a low risk of brain metastasis and have mainly a supratentorial disease site, mainly showing respiratory symptoms and neurological symptoms.
