摘要
衰弱是老年人失能的前兆,患病率高,危害较大,与老年心脑血管病密切相关,是其独立的危险因素。目前对于衰弱尚无有效的特异性治疗手段和成熟的干预模式。我们从社区实际出发,借鉴多学科干预模式,运用“中医全科-专科联合门诊”模式,对心脑血管疾病老年衰弱患者进行干预。该模式在提高诊疗效率、提升慢病管理的水平和质量、提高专科医师和全科医师水平等方面体现出初步的作用,为社区医疗机构多学科干预模式治疗心脑血管疾病老年衰弱患者提供了新的思路,之后需要在临床实践中进一步探讨其作用。
Asthenia is the precursor of disability in the elderly,with high prevalence and great harm.It is closely related to cardiac-cerebral vascular diseases in the elderly and is an independent risk factor.At present,there is no effective specific treatment and mature intervention model for asthenia.Starting from the reality of the community and drawing lessons from the multi-disciplinary intervention model,the model of“general practitioner-specialist joint clinic of traditional Chinese medicine”is used in this paper to treatment asthenia patients with cardiac-cerebral vascular disease in the elderly.This model has shown a certain effect in improving the efficiency of diagnosis and treatment,the chronic disease management level and quality,and the ability of specialist physicians and general practitioners.It provides a new idea for the multi-disciplinary intervention model of community medical institutions in the treatment of asthenia patients with cardiac-cerebral vascular disease in the elderly,and its application needs to be further explored in clinical practice.
