摘要
目的 探讨程序化细胞死亡因子5(PDCD5)在肺炎支原体肺炎(MPP)患儿血清中的表达情况及与心肌损害的相关性。方法 选取2018年7月—2020年5月本院门急诊就诊的77例MPP患儿,根据患儿疾病严重程度将其分为轻症组(n=55)和重症组(n=22),同时选取60例来本院体检的健康儿童为对照组;收集研究对象一般资料,根据MPP患儿是否发生心肌损害分为心肌损害组(n=25)和非心肌损害组(n=52),检测心肌酶谱指标;采用酶联免疫吸附(ELISA)法检测血清中PDCD5水平;Pearson法分析PDCD5水平与心肌酶谱指标的相关性;受试者工作特征曲线(ROC)评价血清PDCD5水平对MPP患儿并发心肌损害的诊断价值。结果 与对照组比较,MPP轻症和重症组患儿血清中PDCD5水平均上调,差异具有统计学意义(P<0.05),且重症组PDCD5水平比轻症组明显升高(P<0.05);心肌损害组患儿血清PDCD5水平高于非心肌损害组,差异有统计学意义(P<0.05);与对照组相比,心肌损害组和非心肌损害组患儿血清心肌酶谱指标AST、LDH、α-HBDB、CK、CK-MB、hs-cTnI水平均明显升高,且心肌损害组AST、LDH、α-HBDB、CK、CK-MB、hs-cTnI水平高于非心肌损害组,差异均具有统计学意义(P<0.05);MPP患儿血清PDCD5水平与AST、LDH、α-HBDB、CK、CK-MB、hs-cTnI水平均呈正相关(P<0.05);ROC结果显示,血清PDCD5水平预测MPP患儿并发心肌损害的曲线下面积(AUC)为0.883(95%CI:0.790~0.945),对应的敏感度为80.00%,特异度为92.31%。结论 PDCD5在MPP患儿血清中明显高表达,且重症组患儿高于轻症组患儿,心肌损害组患儿高于非心肌损害组患儿,PDCD5可作为预测MPP患儿并发心肌损害的重要血清学指标。
Objective To investigate the expression of programmed cell death 5(PDCD5) in the serum of children with Mycoplasma pneumoniae pneumonia(MPP) and its correlation with myocardial damage. Methods From July 2018 to May 2020, 77 children with MPP who were admitted to the outpatient and emergency department of our hospital were selected and divided into a mild group(n=55) and a severe group(n=22) according to the severity of the disease. At the same time, another 60 healthy children who came to our hospital for physical examination were selected as the control group. The general data of the study subjects were collected. According to whether the MPP children have myocardial damage, they were divided into the myocardial damage group(n=25) and the non-myocardial damage group(n=52). Myocardial zymogram indexes were detected and the level of PDCD5 in serum was detected by enzyme-linked immunosorbent assay(ELISA). The Pearson method was used to analyze the correlation between PDCD5 level and myocardial zymogram indexes. The receiver operating characteristic curve(ROC) was used to evaluate the value of serum PDCD5 level in the diagnosis of myocardial damage in children with MPP. Results Compared with the control group, the level of PDCD5 in the serum of MPP children in the mild and severe groups were up-regulated, and the difference was statistically significant(P<0.05), and the level of PDCD5 in the severe group was significantly higher than that in the mild group(P<0.05);the level of serum PDCD5 of children in the myocardial damage group was higher than that in the non-myocardial damage group,and the difference was statistically significant( P < 0. 05). Compared with the control group,the serum levels of AST,LDH,α-HBDB,CK,CK-MB,and hs-cTnI in the myocardial damage group and the non-myocardial damage group were significantly increased,and the levels of AST,LDH,α-HBDB,CK,CK-MB,and hs-cTnI in the myocardial damage group were higher than those in the non-myocardial damage group,and the differences were statistically significant( P < 0. 05);Serum PDCD5 levels were positively correlated with AST,LDH,α-HBDB,CK,CK-MB,and hs-cTnI levels in children with MPP( P < 0. 05). ROC results showed that the area under the curve( AUC) of serum PDCD5 level in predicting myocardial damage in children with MPP was 0. 883( 95% CI: 0. 790-0. 945),the corresponding sensitivity was 80. 00%,and the specificity was 92. 31%. Conclusion PDCD5 is highly expressed in the serum of children with MPP,and children in the severe group have higher PDCD5 than those children in the mild group,and children in the myocardial damage group have higher PDCD5 than children in the non-myocardial damage group. PDCD5 can be used as an important serological index to predict myocardial damage in children with MPP.
