摘要
甲状腺结节发病率较高,一般无明显临床症状,且结节性质良性居多,少数为恶性。其发病机制尚不明确,与生活习惯、环境因素、遗传因素等密切相关。中医学将其归为“瘿病”范畴,其病机为气滞、血瘀、痰浊阻塞于颈前。玄府为气机升降之门户、气液通行之道路、神机运转之枢轴、阴阳调和之孔窍。玄府畅通,机体气血津液得以正常运行,则神机可正常运转。甲状腺中钠碘同向转运蛋白(sodium iodine symporter,NIS)的表达,与甲状腺结节的形成有一定关联,且与中医学玄府的功能有一定的相似性。玄府郁闭为NIS表达失常的重要变化,开通玄府是防治甲状腺结节的基本治则,包括中药内服开玄法、中药外敷开玄法、针刺温灸通络开玄法。
The incidence of thyroid nodules is high,and there are generally no obvious clinical symptoms.Most of the nodules are benign and a few are malignant.Its pathogenesis is still unclear,and it is closely related to living habits,environmental factors and genetic factors.Traditional Chinese medicine classifies it as"gall disease",which is blocked in front of the neck due to Qi stagnation,blood stasis and phlegm.Xuanfu is the gateway of Qi movement,the road for gas and liquid to pass through,the pivot of divine machine operation,and the orifice of Yinyang harmony.Xuanfu is unblocked,and the body’s Qi,blood and body fluid can run normally,so the divine machine can run normally.The expression of sodium iodide symporter(NIS)in thyroid gland is related to the formation of thyroid nodules,and is similar to the function of Xuanfu in traditional Chinese medicine.Xuanfu Yubi is an important change of NIS expression disorder,and Xuanfu is the basic treatment principle for preventing and treating thyroid nodules,including oral administration of traditional Chinese medicine,external application of traditional Chinese medicine,acupuncture and warm moxibustion dredging collaterals.
