通过单操作孔胸腔镜下手术治疗早期非小细胞肺癌患者的效果 被引量:1

Efficacy of Single-port Video-assisted Thoracoscopic Surgery in the Treatment of Early Stage Non-small Cell Lung Cancer

摘要 目的 对早期非小细胞肺癌患者在临床中采用单操作孔胸腔镜下手术(VATS)治疗的效果进行探讨。方法 将2019年1月至2020年1月于我院收治的110例早期非小细胞肺癌患者平均分成两组,每组55例。常规组接受胸腔镜辅助小切口(VAMT)肺叶切除术治疗,给予患者气管插管全身麻醉,引导患者采取健侧卧位,做好消毒铺巾工作,于患者患侧7、8肋间作切口,作为观察孔,置入胸腔镜,于腋中线第8肋间作切口,作为观察孔,在直视下将患者肿瘤切除,并清扫淋巴。试验组接受VATS治疗,给予患者气管插管全身麻醉,引导患者采取健侧卧位,做好消毒铺巾工作,胸腔镜置入方法与对照组相同,于腋前线第4肋间做操作孔,拉开肌肉和筋膜,以胸腔镜显示器为导视,切除肿瘤,并清扫淋巴,两组术后均给予镇痛泵镇痛处理,对比分析2种治疗方案对早期非小细胞肺癌患者的治疗效果。结果 试验组和常规组的术后并发症发生情况和术前疼痛程度对比组间无差异(P>0.05);相比常规组,试验组患者的相关手术指标以及术后1~3 d疼痛程度恢复较好,组间差异有统计学意义(P<0.05)。结论 VATS可有效改善早期非小细胞肺癌患者术后的疼痛程度,提高患者的术后指标恢复情况,治疗效果明显。 Objective To investigate the clinical effect of single-port video-assisted thoracoscopic surgery(VATS) in patients with early non-small cell lung cancer. Methods A total of 110 patients with early-stage non-small cell lung cancer admitted to our hospital from January 2019 to January 2020 were divided into two groups, 55 cases in each group. Group received conventional thoracoscope assisted small incision(VAMT) treatment,lung resection for patients with tracheal intubation general anesthesia, guide the patients to take the healthy side lies, do a good job, disinfection shop towels in patients suffering from lateral incision, 7, 8 intercostals do to observe hole, placing thoracoscope, in axillary midline intercostals do incision, 8 for the observation hole, the sufferer under direct tumor resection, Lymph nodes were also dissected. The experimental group received single hole under the thoracoscope surgery(VATS) operation treatment, giving patients with tracheal intubation general anesthesia, guide the patients to take the healthy side lies, do a good job, disinfection shop towel suture placement method is the same as the control group, in axillary the front rib between 4 holes, making operation away muscle and fascia, with thoracoscope displays for advertising, removal of the tumor, and clean up lymphatic.Both groups were treated with analgesic pump after operation, and the therapeutic effects of the two treatments on patients with early non-small cell lung cancer were compared and analyzed. Results There was no significant difference in postoperative complications and preoperative pain between the experimental group and the conventional group(P>0.05). Compared with the conventional group, the relevant surgical indicators and the degree of pain recovered better in the experimental group at 1-3 days after operation, and the differences between the groups were significant(P<0.05).Conclusion Single-port thoracoscopic surgery has obvious effect, which can effectively improve the postoperative pain degree of patients with early non-small cell lung cancer, improve the recovery of postoperative indicators of patients.
