白色念珠菌引起抗菌药物相关性腹泻的临床检验特点分析

摘要 目的:分析白色念珠菌引起抗菌药物相关性腹泻的临床检验特点。方法:回顾性分析2020年1月-2022年6月山东省济南市长清区人民医院收治的抗菌药物相关性腹泻患者60例临床资料,根据患者抗菌药物相关性腹泻发生原因与白色念珠菌是否有关进行分组,与白色念珠菌无关患者为对照组,与白色念珠菌有关患者为观察组,各30例。对两组患者行药敏试验,分析患者对不同药物的敏感情况。比较两组患者空腹血糖、血钾、血钠及清蛋白水平。结果:观察组血钾、血钠、空腹血糖水平高于对照组,清蛋白水平低于对照组,差异有统计学意义(P<0.05)。两组患者对氟康唑、伊曲康唑、伏立康唑、两性霉素B、氟胞嘧啶的敏感性较高,且观察组高于对照组,差异有统计学意义(P<0.05);两组患者对万古霉素、呋喃妥因敏感性较低,且观察组低于对照组,差异有统计学意义(P<0.05);两组患者青霉素G及左氧氟沙星敏感性比较,差异有无统计学意义(P>0.05)。结论:对抗菌药物相关性腹泻患者早期给予粪便标本的细菌培养和鉴定以明确菌种,有助于指导临床合理使用抗菌药物,提高抗菌药物使用的安全性,对白色念珠菌引起的抗菌药物相关性腹泻的诊疗具有重要意义。 Objective:To analyze the clinical testing characteristics of antimicrobial-associated diarrhea caused by Candida albicans.Methods:The clinical data of 60 patients with antimicrobial-associated diarrhea who were treated in Changqing District People's Hospital of Jinan City from January 2020 to June 2022 were retrospectively analyzed.Patients were divided into groups according to whether the causes of antibacterial-associated diarrhea were related to Candida albicans.Patients unrelated to Candida albicans were included into control group and patients related to Candida albicans were included into the observation group,with 30 cases in each group.Drug sensitivity test was performed for the patients to analyze the sensitivity of patients in the two groups to different drugs.The fasting blood glucose index,blood potassium level,blood sodium level and albumin level were compared between the two groups.Results:The blood potassium level,blood sodium level and fasting blood glucose level in the observation group were higher than those in the control group,and the albumin level in the observation group was lower than that in the control group,and the differences were statistically significant(P<0.05).The sensitivity of patients in the two groups to fluconazole,itraconazole,voriconazole,amphotericin B and flucytosine was higher,and the sensitivity in the observation group was higher than that in the control group,and the differences were statistically significant(P<0.05).The sensitivity of patients in the two groups to vancomycin and nitrofurantoin was low,and the sensitivity in the observation group was lower than that in the control group,and the differences were statistically significant(P<0.05).There was no statistically significant difference in the sensitivity to penicillin G and levofloxacin between the two groups(P>0.05).Conclusion:Bacterial culture and identification of fecal specimens at early stage for patients with antimicrobial-associated diarrhea can determine the strains and help to guide the rational use of antimicrobials in clinical practice and improve the safety of antimicrobials used.It is of great significance for the diagnosis and treatment of antimicrobial-associated diarrhea caused by Candida albicans.
