摘要
目的 探讨饮食护理对肝胆外科术后患者恢复的影响。方法 选择2018年3月至2020年1月期间在本院肝胆外科收治的88例住院患者作为研究对象,根据护理模式的不同,分为个体饮食组和对照组,分别纳入患者46例和42例。对照组按照肝胆外科围手术期护理要求进行常规护理,个体饮食组在对照组的基础上增加个体化饮食护理,观察两组术后恢复效果,并对比两组并发症风险、营养水平与术后疼痛水平和康复水平。结果 个体饮食组术后排气时间(29.81±7.56)h、术后早期疼痛评分(4.26±1.57)分、术后排便时间(31.02±9.87)h、术后下床活动时间(23.03±7.58)h和术后住院时间(6.58±1.68)d显著低于对照组[(38.03±10.57)h、(5.98±1.87)分、(40.02±11.57)h、(35.15±10.74)h、(9.23±2.04)d],差异具有统计学意义(P <0.05)。个体饮食组术后并发症总发生率(8.70%,4/46),显著低于对照组(23.81%,10/42),差异具有统计学意义(P <0.05)。个体饮食组血红蛋白平均指数为(113.02±13.25)g/L、总蛋白平均指数为(66.42±6.38)g/L、转铁蛋白平均指数为(2.55±0.62)g/L、前清蛋白水平平均指数为(38.24±4.11)g/L,各个数值均高于对照组,差异具有统计学意义(P <0.05)。结论 肝胆外科术后根据患者个体特征给予饮食护理,可有效减轻术后早期疼痛,降低并发症风险,有助于缩短术后恢复进程,改善患者预后与营养水平。
Objective To explore the impact of dietary nursing on the recovery of patients after hepatobiliary surgery.Methods A total of 88 inpatients admitted to the department of hepatobiliary surgery of our hospital from March 2018 to January 2020 were selected as research subjects.According to different nursing modes,they were divided into individual diet group and control group,including 46 patients and 42 patients,respectively.The control group received routine nursing according to the requirements of perioperative nursing of hepatobiliary surgery,and the individual diet group increased individual diet nursing on the basis of control group.The postoperative recovery effect of the two groups was observed,and the risk of complications,nutritional level,postoperative pain level and rehabilitation level of the two groups were compared.Result The individual diet group postoperative exhaust time(29.81±7.56)h,postoperative early pain score(4.26±1.57)points,postoperative defecation time(31.02±9.87)h,postoperative ambulation time(23.03±7.58)h and postoperative hospital stay(6.58±1.68)d were significantly lower than those in the control group[(38.03±10.57)h,(5.98±1.87)points,(40.02±11.57)h,(35.15±10.74)h,(9.23±2.04)d],the differences were statistically significant(P<0.05).The total incidence of postoperative complications in individual diet group(8.70%,4/46)was significantly lower than that in control group(23.81%,10/42),the difference was statistically significant(P<0.05).The levels of hemoglobin(113.02±13.25)g/L,total protein(66.42±6.38)g/L,transferrin(2.55±0.62)g/L and proalbumin(38.24±4.11)g/L in individual diet group were higher than those in control group,the differences were statistically significant(P<0.05).Conclusion Dietary nursing after hepatobiliary surgery according to the individual characteristics of patients can effectively relieve early postoperative pain,reduce the risk of complications,help to shorten the postoperative recovery process,improve the prognosis and nutritional level of patients.
