摘要
目的:探讨腹横筋膜阻滞对腹腔镜结肠癌根治术患者术中循环波动的影响.方法:回顾性分析,选取本院2019年4月至2020年3月收治的40例行腹横筋膜阻滞联合全身麻醉的腹腔镜结肠癌根治术患者作为观察组,选取本院2018年4月至2019年2月收治的42例行常规全身麻醉的腹腔镜结肠癌根治术患者作为对照组.记录并比较诱导结束时(T1)、手术10min(T2)、缝合时(T3)、出手术室时(T4)4个时间点两组患者心率、收缩压及舒张压.结果:T2、T3、T4两组心率、收缩压、舒张压均较T1升高,而观察组变化幅度较对照组低;组别、时间点、组别·时间点比较,差异有统计学意义(P<0.05).结论:腹横筋膜阻滞用于腹腔镜结肠癌根治术患者术中,可控制术中循环波动,降低手术风险,值得临床广泛应用.
Objective:To investigate the influence of transverse abdominal fascia block on intraoperative circulatory fluctuations in patients undergoing laparoscopic radical resection of colon cancer.Methods:A retrospective analysis was performed,40 patients who underwent laparoscopic radical resection of colon cancer surgery with transverse abdominal fascia block combined with general anesthesia treated in our hospital from April 2019 to March 2020 were selected as the observation group,42 patients who underwent conventional general anesthesia with laparoscopic radical resection of colon cancer from April 2018 to February 2019 were selected as the control group.The hemodynamic parameters,such as heart rate,systolic blood pressure and diastolic blood pressure were compared between the two groups at dfferent time points at the end of anesthesia induction(T1),10min of operation(T2),during suture(T3),and leaving operating room(T4).Results:At T2,T3,and T4,the heart rate,systolic blood pressure and diastolic blood pressure levels of the two groups were increased compared with those at T1,but the magnitude of change in the observation group was lower than that of the control group.Between the two groups,time points,and group·time points,there were significantly statistical difference(P<0.05).Conclusion:Transverse abdominal fascia block is used in patients undergoing laparoscopic radical resection of colon cancer,which can control intraoperative circulatory fluctuations and reduce surgical risks.It is worthy of clinical application.
