摘要
目的:探讨胎儿先天性心脏病(congenital heart disease,CHD)超声筛查中心脏横断面序贯联合肺动脉分支切面扫查的应用价值。方法:选取2018年3月—2020年3月样本医院收治的280例疑似胎儿心脏异常孕妇作为研究对象,所有孕妇均经产前心脏横断面序贯联合肺动脉分支切面扫查和单纯心脏横断面序贯筛查,随访至妊娠结束。以引产胎儿尸检和新生儿心脏检查结果为“金标准”,记录心脏横断面序贯联合肺动脉分支切面扫查和单纯心脏横断面序贯筛查胎儿CHD检出情况,比较分析心脏横断面序贯联合肺动脉分支切面扫查和单纯心脏横断面序贯敏感度、特异度、准确度。结果:根据“金标准”,心脏横断面序贯联合肺动脉分支切面扫查和单纯心脏横断面序贯检出率分别为98.48%、68.18%,差异有统计学意义(P<0.05);心脏横断面序贯联合肺动脉分支切面扫查敏感度、准确度均高于单纯心脏横断面序贯,差异有统计学意义(χ^(2)=23.220、7.843、22.688,P<0.05);心脏横断面序贯联合肺动脉分支切面扫查特异度与单纯心脏横断面序贯比较,差异无统计学意义(P>0.05)。结论:心脏横断面序贯联合肺动脉分支切面扫查胎儿CHD具有较高敏感度,筛查价值较高。
Objective:To explore the application value of sequential cross-section of the heart combined with the branch section of the pulmonary artery in the ultrasound screening of fetal congenital heart disease(CHD).Methods:A total of 280 pregnant women with suspected fetal cardiac abnormalities admitted to the sample hospital from March 2018 to March 2020 were selected as the research objects.All pregnant women underwent sequential screening of prenatal cardiac cross section combined with pulmonary artery branch section scan and sequential screening of pure cardiac cross section,and were followed up until the end of pregnancy.The results of induced fetal autopsy and neonatal cardiac examination were taken as the“gold standard”,the detection of fetal CHD was recorded in the sequential cross-sectional view of the heart combined with the pulmonary artery branch section scan and the sequential screening of the fetus with the simple cardiac cross-section.The sensitivity,specificity and accuracy of sequential cardiac cross-sectional scan combined with pulmonary artery branch view and simple cardiac cross-sectional sequential scan were compared and analyzed.Results:According to the“gold standard”,the detection rates of sequential cross-sectional view of the heart combined with pulmonary artery branch view and sequential cross-sectional view of the heart alone were 98.48%and 68.18%,respectively,and the difference was statistically significant(P<0.05).The sensitivity and accuracy of the sequential cardiac cross-section combined with the pulmonary artery branch view were higher than those of the pure cardiac cross-section,and the difference was statistically significant(χ^(2)=23.220,7.843,22.688,P<0.05).There was no statistically significant difference in the specificity between the sequential cross-sectional view of the heart combined with the view of the branch of the pulmonary artery and the sequential view of the pure heart cross-section(P>0.05).Conclusion:Cardiac cross-section sequential scan combined with pulmonary artery branch view has high sensitivity and high screening value for fetal CHD.
