中晚期胎儿肝区脐血流分布状态与胎龄及生长指标的相关性 被引量:2

Correlations of liver region distribution of umbilical blood flow at the middle and third trimester with gestational age and growth indicators

摘要 目的分析妊娠中晚期胎儿肝区脐血流分布状态与胎龄及生长指标的相关性。方法对714胎胎龄17~40^(+6)周发育正常胎儿行超声检查,记录胎儿生长指标,包括双顶径、头围、腹围、股骨长及体质量;测量胎儿脐静脉(UV)腹内段(D_(UV))、门静脉窦(PS)起始段(D_(PS))、静脉导管(DV)起始段(D_(DV))内径及其相应最大血流速度V_(UV)、V_(PS)和V_(DV),计算UV、DV、左门静脉(LPV)及右门静脉(RPV)血流量,即Q_(UV)、Q_(DV)、Q_(LPV)、Q_(RPV);分析脐血流相关参数与胎龄及生长指标的相关性。结果胎儿D_(UV)、D_(PS)、D_(DV)及V_(UV)、V_(PS)均与胎龄呈正相关(r=0.907、0.901、0.874、0.393、0.301,P均<0.001),V_(DV)与胎龄无明显相关性(P=0.817);胎儿Q_(UV)、Q_(DV)、Q_(LPV)及Q_(RPV)与胎龄呈正相关(r_(s)=0.904、0.895、0.797、0.761,P均<0.05)。DV分流率与胎龄及双顶径、头围、腹围、股骨长及体质量均呈负相关(r_(s)=-0.302、-0.306、-0.311、-0.332、-0.304、-0.321,P均<0.001),LPV分流率与上述参数均呈正相关(r_(s)=0.245、0.221、0.228、0.236、0.230、0.230,P均<0.001),RPV分流率与上述参数均无明显相关性(P均>0.05)。结论妊娠中晚期胎儿肝区脐血流分布状态与胎龄及生长指标具有一定相关性。 Objective To explore the correlations of distribution status of umbilical blood flow in liver region at the middle and third trimester with gestational age(GA)and growth indicators.Methods Ultrasonography was performed on 714 fetuses with GA with GA of 17—40^(+6) weeks.Fetal growth indicators,including biparietal diameter(BPD),head circumference(HC),abdominal circumference(AC),femur length(FL)and body weight were recorded.Internal diameter(D_(UV),D_(PS),D_(DV))of abdominal segment of umbilical vein(UV),portal sinus(PS)and the beginning segment of ductus venosus(DV),also the corresponding maximum blood flow velocity(V_(UV),V_(PS),V_(DV))were measured,and the volume of blood flow within UV,DV,left portal vein(LPV)and right portal vein(RPV),i.e.Q_(UV),Q_(DV),Q_(LPV) and Q_(RPV) were calculated.The correlations of fetal umbilical blood flow related parameters with GA and growth indicators were analyzed.Results Fetal D_(UV),D_(PS),D_(DV),V_(UV) and V_(PS) were positively correlated with GA(r=0.907,0.901,0.874,0.393 and 0.301,all P<0.001),while no significant correlation of V_(DV) with GA was found(P=0.817).Fetal Q_(UV),Q_(DV),Q_(LPV) and Q_(RPV) were positively correlated with GA(r_(s)=0.904,0.895,0.797 and 0.761,all P<0.05).DV shunt rate were negatively correlated with GA and growth indicators BPD,HC,AC,FL and body weight(r_(s)=-0.302,-0.306,-0.311,-0.332,-0.304,-0.321,all P<0.001),while LPV shunt rate was positively correlated with the above parameters(r_(s)=0.245,0.221,0.228,0.236,0.230,0.230,all P<0.001),but RPV shunt rate was not correlated with the above parameters(all P>0.05).Conclusion Fetal umbilical blood flow distribution in the liver region at the middle and third trimester had certain correlations with GA and growth indicators.
