摘要
目的探讨无创呼吸机联用布地奈德混悬液对慢性阻塞性肺疾病(chronic obstructive pulmonary diseases,COPD)患者肺功能、炎症因子和BORG疲劳量表(Borg scale,BORG)的影响。方法选取浙江省丽水市第二人民医院2019年1月至2021年12月住院治疗的205例COPD患者作为研究对象,采用数字表法随机分为对照组(常规治疗+无创呼吸机)103例和研究组(常规治疗+无创呼吸机+布地奈德混悬液)102例,比较两组患者治疗后的临床疗效、呼吸频率(respiratory rate,RR)、心率(heart rate,HR)、pH值、BORG评分、血气指标、肺功能及炎症因子指标。结果治疗后,研究组总有效率明显高于对照组(P<0.05);研究组BORG评分高于对照组;研究组RR、HR、动脉血二氧化碳分压、氧合指数(oxygenation index,OI)、肿瘤坏死因子(tumour necrosis factor-α,TNF-α)、白细胞介素-8(interleukin-8,IL-8)均低于治疗前和同期的对照组(P<0.05),研究组pH、血氧饱和度(oxygen saturation,SO_(2))、动脉血氧分压(partial pressure of oxygen,PaO_(2))、1s用力呼气容积(forced expiratory volume in one second,FEV_(1))、第1秒用力呼气量占所有呼气量比例(FEV_(1)/FVC)及FEV_(1)%均高于治疗前和同期的对照组(P<0.05)。结论无创呼吸机联用布地奈德混悬液治疗COPD比单纯使用无创呼吸机治疗临床效果更为显著,它能够有效地增加肺通气量,改善血气指标,抑制炎性反应,缓解呼吸困难程度,值得推广应用。
Objective To explore the effect of non-invasive ventilator combined with Pulmicorin on pulmonary function,inflammatory factors and BORG score in patients with chronic obstructive pulmonary disease COPD.Methods A total of 205 COPD patients hospitalized in our hospital from January 2019 to December 2021 were selected as the research subjects and analyzed retrospectively.They were divided into control group(103 cases of routine treatment+non-invasive ventilator)and experimental group(102 cases of routine treatment+non-invasive ventilator).The clinical efficacy,RR,HR,pH,Borg Fatigue Scale(BORG score),blood gas index,Pulmonary function,and evels of inflammation factors were compared between the two groups.Results After treatment,the total effective rate of the experimental group was significantly higher than that of the control group(P<0.05);The BORG score of the experimental group was better than the control group;The RR,HR,arterial blood carbon dioxide partial pressure(PaCO_(2)),oxygenation index(OI),tumor necrosis factor(TNF-α),and interleukin-8(IL-8)in the experimental group were lower than those in the Pre-treatment and concurrent control group(P<0.05);The experimental group pH,blood oxygen saturation(SaO_(2)),arterial oxygen partial pressure(PaO_(2)),1 second forced expiratory volume(FEV_(1)),FEV_(1) to vital capacity ratio(FEV_(1)/FVC)and FEV_(1)%were higher than those in the Pre-treatment and concurrent control group(P<0.05).Conclusion Non-invasive ventilator combined with in the treatment of COPD is more effective than non-invasive ventilator alone.It can effectively increase lung ventilation,improve blood gas index,inhibit inflammation,and reduce the degree of dyspnea.It is worth popularizing and applying.
