摘要
目的:分析幽门螺杆菌(Hp)粪便抗原(HpSA)检测在诊断患者Hp感染中的应用价值。方法:选取2019年1月-2022年1月北京和睦家医院收治的因上消化道症状就诊的患者100例作为研究对象,采用快速尿素酶试验和组织切片染色联合检测,以此作为Hp的诊断金标准,并使用ELISA法检测粪便样本中的HpSA,评价HpSA的特异度、灵敏度和准确度。比较HpSA与13C尿素呼气试验结果。结果:100例患者中Hp阳性64例,其中HpSA阳性60例、HpSA阴性4例,和金标准比较,灵敏度93.8%(60/64),特异度94.4%(34/36),准确度94.0%(94/100)。HpSA与13C尿素呼气试验灵敏度分别为81.8%、90.9%;特异度为93.9%、98.0%;准确度为91.7%、96.7%,两种方法比较,差异无统计学意义(P>0.05)。结论:HpSA检测具有操作简单、便捷、非入侵性的优势,在Hp感染诊断过程中具有可靠性、安全性和准确性。
Objective:To analyze the application value of Helicobacter pylori(Hp)stool antigen(HpSA)detection in the diagnosis of Helicobacter pylori infection in patients.Methods:A total of 100 patients with upper gastrointestinal symptoms who were admitted to Beijing United Family Hospital from January 2019 to January 2022 were selected as the research subjects,and the combined detection of rapid urease test and tissue section staining was used as the diagnosis golden standard of Hp.HpSA in stool samples was detected by ELISA,and the specificity,sensitivity and accuracy of HpSA were evaluated.HpSA and 13C urea breath test results were compared.Results:Among the 100 patients,64 cases were positive for Hp,including 60 cases positive for HpSA and 4 cases negative for HpSA.Compared with the gold standard,the sensitivity was 93.8%(60/64),the specificity was 94.4%(34/36),and the accuracy was 94.0%(94/100).For HpSA and 13C urea breath test,the sensitivity was 81.8%and 90.9%,the specificity was 93.9%and 98.0%,the accuracy was 91.7%and 96.7%.There was no significant difference between the two methods(P>0.05).Conclusion:HpSA detection has the advantages of simple operation,convenience and non-invasiveness,and it is reliable,safe and accurate in the diagnosis of Hp infection.
