摘要
目的探讨改良胃空肠Roux-en-Y吻合术在胃肠手术中的临床应用效果。方法方便选取2020年3月—2021年1月山东省单县中心医院收治的66例胃肠疾病患者为研究对象,随机分为采用改良胃空肠Roux-en-Y吻合术的观察组(n=33),与采用标准常规胃空肠Roux-en-Y吻合术的对照组(n=33)。比较两组患者手术时间、术中出血量、术后疼痛情况、术后生活质量及并发症发生率。结果观察组手术时间(178.09±12.47)min、术中出血量(50.52±11.04)mL、术后疼痛评分(2.18±1.09)分均显著低于对照组(239.14±17.29)min、(189.46±12.63)mL、(5.31±1.94)分,差异有统计学意义(P<0.05)。观察组术后并发症发生率为6.06%,显著低于对照组的36.36%,差异有统计学意义(χ^(2)=9.970,P<0.05)。观察组术后健康感觉(83.17±8.14)分、心理状况(87.04±4.74)分、睡眠质量(82.51±4.24)分、功能恢复(80.14±4.23)分生活质量评分显著高于对照组,差异有统计学意义(P<0.05)。结论改良胃空肠Roux-en-Y吻合术能够缩短治疗时间、减少手术对患者带来的伤害,减少术后并发症发生率,减少对患者生存质量的影响,值得推广。
Objective To investigate the clinical application effect of modified gastrojejunostomy Roux-en-Y anastomosis in gastrointestinal surgery.Methods A total of 66 patients with gastrointestinal diseases admitted to the ShanXian Central Hospital of Shandong Province from March 2020 to January 2021 were conveniently selected as the research subjects.They were randomly divided into observation group(n=33)with modified gastrojejunostomy Rouxen-Y anastomosis,and control group(n=33)with standard conventional gastrojejunostomy Roux-en-Y anastomosis.The operation time,intraoperative blood loss,postoperative pain,postoperative quality of life and the incidence of complications were compared between the two groups.Results The operation time(178.09±12.47)min,intraoperative blood loss(50.52±11.04)mL and postoperative pain score(2.18±1.09)points of the observation group were significantly lower than those of the control group(239.14±17.29)min,(189.46±12.63)mL,(5.31±1.94)points,the difference was statistically significant(P<0.05).The incidence of postoperative complications in the observation group was 6.06%,which was significantly lower than 36.36%in the control group,and the difference was statistically significant(χ^(2)=9.970,P<0.05).The postoperative quality of life scores of the observation group in terms of feeling of health(83.17±8.14)points,psychological status(87.04±4.74)points,sleep quality(82.51±4.24)points and functional recovery(80.14±4.23)points were significantly higher than those of the control group,the difference was statistically significant(P<0.05).Conclusion The modified gastrojejunostomy Roux-en-Y anastomosis can shorten the treatment time,reduce the injury caused by the operation,reduce the postoperative incidence of complications,and reduce the impact on the quality of life of the patients,which is worthy of promotion.
