摘要
《伤寒论》成书于东汉末年,伤寒是那时发热类疾病的统称。从现代医学角度分析,绝大多数发热性疾病属于感染性疾病。因此,张机(字仲景)所论伤寒应属于广义伤寒。后世学者对六经及六经辨证的本质多有争论,参考《伤寒论》成书时的理论局限,笔者认为六经是按同名经络命名的六组症候群,这些症状的产生可能与感染性疾病相关,也可能与感染性疾病无关,六组症候群之间并没有严格的时间上的先后顺序,因此并不存在严格的六经传变规律。虽然六经辨证的本质与感染性疾病密切相关,也认识到感染性疾病的症状、病程有一定的规律,但其对感染性疾病的认识和治疗方法都有历史局限性。文章参考《伤寒论》,结合现代医学中感染性疾病的一般规律和理论,解读关于广义与狭义伤寒、六经辨证本质、寒温之争等问题。
Shanghan Lun(《伤寒论》)was written at the end of the eastern Han dynasty,exogenous febrile disease was a general name for febrile diseases.From the perspective of modern medicine,most febrile diseases were infectious diseases.Therefore,the author thinks that exogenous febrile disease discussed by ZHANG Zhongjing belongs to generalized exogenous febrile disease.Later generations of physicians continue to argue about the essence of the Six Meridians and their differentiation.Referring to the theoretical origin of Shanghan Lun,the author thinks that the Six Meridians are six groups of symptoms named after the meridians of the same name,and these symptoms can be related to infectious diseases,or it may have nothing to do with infectious diseases.There is no strict chronological order between the six groups of symptoms,so there is no strict law of transmission and change of the Six Meridians.Although the essence of syndrome differentiation of Six Meridians is closely related to infectious diseases,it is also recognized that the symptoms and course of infectious diseases have certain rules,its understanding of infectious diseases and treatment methods have historical limitations.The article refers to Shanghan Lun,combined with the general rules and theories of infectious diseases in modern medicine,to interpret issues such as typhoid fever in the broad and narrow senses,the nature of Six Meridians,and the dispute between cold and warm.
