摘要
目的:探究肝癌患者血清CA153、甲胎蛋白(AFP)、铁蛋白(Ferr)表达水平和临床意义。方法:选取32例乙肝相关肝癌(肝癌组)、31例慢性乙肝(肝炎组)、27例乙肝肝硬化(肝硬化组)及同期35名健康体检者(对照组)进行回顾性分析,比较各组患者临床基本资料和血清CA153、AFP、Ferr水平,分析临床病理特征及肝癌的影响因素。结果:肝癌组与肝炎组和肝硬化组在部分临床表现、血常规指标和生化指标比较,差异均有统计学意义(P<0.05);比较各组血清CA153、AFP、FERRITIN水平,其中肝癌组高于慢性乙肝组、肝硬化组和对照组(P<0.05),肝硬化组高于慢性乙肝组(P<0.05);血清CA153、AFP、FERRITIN水平与慢性乙肝、乙肝肝硬化及乙肝相关原发性肝癌阶段的AST、ALT、GLO、DBIL、ALP、GGT呈正相关(P<0.05),与ALB呈负相关(P<0.05)),并随肿瘤分期、Child-Pugh分级及肿瘤数量提升而升高(P<0.05)),肿瘤数量为1~2个的患者血清CA153、AFP、FERRITIN水平分低于肿瘤数量≥3个的患者(P<0.05);血清CA153、AFP、FERRITIN均为肝癌的影响因素(P<0.05)。结论:肝癌患者血清CA153、AFP、FERRITIN水平较高,与肝功能指标、TNM分期、Child-Pugh分级和肿瘤数量密切相关。
Objective:To explore the expressions and clinical significance of serum carbohydrate antigen 153(CA153),alpha fetoprotein(AFP)and ferritin(Ferr)in patients with liver cancer.Methods:32 patients with hepatitis B-related liver cancer,31 patients with chronic hepatitis B,27 patients with hepatitis B cirrhosis and 35 healthy controls were enrolled for retrospective analysis.The clinical basic data and serum levels of CA153,AFP and Ferr of the four groups were compared,and the clinicopathological characteristics and influencing factors of liver cancer were analyzed.Results:There were significant differences in some clinical manifestations,blood routine indicators and biochemical indicators among liver cancer group,hepatitis group and cirrhosis group(P<0.05).The levels of serum CA153,AFP and Ferr were higher in liver cancer group than those in chronic hepatitis B group,cirrhosis group and healthy control group(P<0.05),and the levels were higher in cirrhosis group than those in chronic hepatitis B group(P<0.05).Serum CA153,AFP and Ferr were positively correlated with AST,ALT,GLO,DBIL,ALP and GGT in chronic hepatitis B,hepatitis B cirrhosis and hepatitis B related liver cancer(P<0.05),and were negatively correlated with ALB(P<0.05),and the levels were increased with the improvements of TNM stage,Child-Pugh grade and the number of tumor(P<0.05),and the levels in patients with 1-2 tumors were lower than those with more than 3 tumors(P<0.05).Serum CA153,AFP and Ferr were the influencing factors of liver cancer(P<0.05).Conclusion:Serum CA153,AFP and Ferr are highly expressed in patients with liver cancer,and they are related to liver function indicators,TNM stage,Child-Pugh grade and tumor number.
