摘要
目的探究五步快速复位法在治疗肩关节前脱位的应用效果分析。方法选取2021年1月至12月兰溪市中医院收治的前肩关节脱位患者54例,均采用五步快速复位法治疗,观察并分析患者治疗前、中、后疼痛情况,以及一次性复位成功率、医疗费用、复位时间和并发症情况,并在治疗后随访3个月,观察其疗效及患者肩关节功能恢复情况,采用多因素Logistic回归分析影响是否一次复位的主要因素。结果治疗中患者疼痛感较治疗前有所提高,治疗后疼痛感较治疗前下降,整体上疼痛感呈下降趋势,差异有统计学意义(P<0.05);患者治疗后2个月及治疗后3个月肩关节功能恢复情况均较治疗后1个月有所提高,且整体恢复情况呈上升趋势,差异有统计学意义(P<0.05);采用五步快速复位法治疗,复位时间短、治疗费用低且一次性复位成功率达到88.89%;经Pearson相关性分析,治疗后视觉模拟评分(visual analog scale,VAS)与复位时间均与治疗3个月后患者肩关节功能恢复状况呈负相关,差异有统计学意义(P<0.05);随访3个月内,患者均未发生腋神经损伤及骨折等并发症;单因素分析显示,年龄及就医时间为影响是否一次复位的因素;多因素分析显示,年龄为影响是否一次复位的主要因素。结论采用五步快速复位法治疗肩关节脱位临床效果较好且一次性复位率高,医疗费用少。
Objective To explore the application effect of five-step rapid reduction method in the treatment of anterior shoulder dislocation.Methods A total of 54 patients with anterior shoulder dislocation who were treated in Lanxi Hospital of Traditional Chinese Medicine from January to December 2021 were selected and treated with five-step rapid reduction method.The pain before,during and after treatment,and the success rate of one-time reduction were observed and analyzed,medical expenses,reduction time and complications,and were followed up for 3 months after treatment to observe the curative effect and the recovery of shoulder joint function of the patients,and multivariate Logistic regression analysis was used to analyze the main factors affecting whether a reset was performed.Results During the treatment,the pain of the patients was improved compared with that before the treatment,and the pain decreased after the treatment,and the overall pain decreased,the difference was statistically significant(P<0.05);compared with the first month,the recovery of shoulder joint function of the patients in the second and third months after treatment was improved,and the overall recovery was on the rise,the difference was statistically significant(P<0.05);the five-step rapid reduction method has a short repositioning time,low treatment cost and a one-time repositioning success rate of 88.89%;according to the Pearson correlation analysis,the visual analog scale(VAS)score and reduction time after treatment were negatively correlated with the recovery of shoulder joint function after 3 months of treatment,the difference was statistically significant(P<0.05);during the 3-month follow-up,no complications such as axillary nerve injury and fracture occurred in all patients;univariate analysis showed that age and medical treatment time were the factors affecting whether a reduction was performed once;multivariate analysis showed that age was the main factor affecting whether a reduction was performed.Conclusion The five-step rapid reduction method for the treatment of shoulder dislocation has a good clinical effect,and the one-time reduction rate is high and the medical cost is low.
