摘要
目的:探究2型糖尿病患者葡萄糖在目标范围内时间(TIR)与合并大血管病变的相关性。方法:选取2020年1月至2020年12月于某院内分泌科住院治疗的2型糖尿病患者78例为研究对象,根据有无合并大血管病变分为大血管病变组(50例)和对照组(28例)。比较2组患者一般资料(年龄、性别、病程、高血压病史、BMI等)、实验室检测结果(HbA_(1c)、血脂指标)及持续葡萄糖监测系统相应数据,采用Logistic回归模型分析糖尿病合并大血管病变的影响因素。结果:2组患者间性别、BMI、舒张压、HbA 1c、HDL-C、有无高血压病史、低血糖发生情况、血糖(最高值、最低值、平均值)比较,差异无统计学意义(P>0.05);大血管病变组年龄、糖尿病病程、收缩压、LDL-C均高于对照组,TIR短于对照组,差异有统计学意义(P<0.05)。Logistic回归分析后显示TIR是糖尿病合并大血管病变的保护因素(OR=0.950,95%CI=0.911~0.990,P=0.014);糖尿病病程、LDL-C是糖尿病合并大血管病变的危险因素[OR=1.210、2.127,95%CI=(1.005~1.457)和(1.037~4.367),P=0.045和0.040]。结论:TIR为糖尿病合并大血管病变的保护因素,临床上可通过提高TIR延缓糖尿病大血管病变的发生和发展。
Objective:To research the correlation of time in range(TIR)and diabetic macrovascular lesions in patients with type 2 diabetes.Methods:A total of 78 patients with type 2 diabetes were gathered in a hospital from January 2020 to December 2020,and were divided into two groups according to the presence of diabetic macrovascular lesions(case group,50 cases)or absence of diabetic macrovascular lesions(control group,28 cases).The general data,laboratory data,and corresponding data of the CGMS of the two groups were analyzed and compared,and the influencing factors of diabetic macrovascular lesions were analyzed by Logistic regression analysis.Results:There were no significant differences between the two groups in terms of gender composition ratio,BMI,diastolic blood pressure(DBP),HbA1c,HDL-C,history of hypertension,hypoglycemia,as well as maximum,minimum and average blood glucose(P>0.05).There were statistically significant differences in age,duration of diabetes,systolic blood pressure(SBP),low density lipoprotein-cholesterol(LDL-C)and TIR between the two groups(P<0.05).Logistic regression analysis showed that TIR was a protective factor for diabetic macrovascular lesions(OR=0.950,95%CI=0.911~0.990,P=0.014),and duration of diabetes and LDL-C were risk factors for diabetic macrovascular lesions(OR=1.210 and 2.127,95%CI=1.005~1.457 and 1.037~4.367,P=0.045 and 0.040).Conclusion:TIR is closely related to the occurrence of diabetic macrovascular lesions.Improving TIR can delay the occurrence and development of diabetes macroangiopathy.
