摘要
目的探讨临床药师网格化实时监控对医院辅助用药合理应用的影响。方法确定本研究中接受干预的10种临床治疗辅助用药,为每一种辅助用药建立相应的合理应用标准。2020年1~6月采用单纯的辅助用药专项点评干预,2020年7~12月在辅助用药专项点评的基础上给予临床药师网格化实时监控,抽取2020年1~6月与7~12月两个时间段的辅助用药病历各800例(每个时间段的每种辅助用药病历80例),对两个时间段病历中的辅助用药使用情况做比较。结果实施临床药师网格化实时监控后,住院患者辅助用药的不合理使用现象(无适应证用药、溶媒不正确、超用法用量、疗程过长、禁忌证等)显著减少,该10种临床治疗辅助用药的销售金额占比(10种临床治疗辅助用药销售金额/全院药品消耗总金额)由2020年1~6月的9.88%下降为2020年7~12月的7.05%。结论与单纯的辅助用药专项点评干预相比,实施临床药师网格化实时监控后,医院临床各科室辅助用药的使用更加趋于合理。
Objective To explore the effect of grid real-time monitoring of clinical pharmacists on the rational application of adjuvant drugs in hospitals.Methods 10 kinds of clinical adjuvant drugs that received intervention in this study were determined,and the corresponding reasonable application standards were established for each kind of adjuvant drug.From January to June 2020,the intervention of simple special comments on auxiliary drugs was adopted,and from July to December 2020,the grid real-time monitoring was given to clinical pharmacists on the basis of special comments on auxiliary drugs,and 800 auxiliary drug medical records(80 auxiliary drug medical records in each time period)were selected from January to June and July to December 2020.The use of adjuvant drugs in the medical records of the two periods was compared.Results After the implementation of grid real-time monitoring by clinical pharmacists,the irrational use of adjuvant drugs in hospitalized patients(medication without indications,incorrect solvent,excessive dosage,long course of treatment,contraindications,etc.)was significantly reduced,and the proportion of the sales amount(sales amount of 10 kinds of auxiliary drugs for clinical treatment/total amount of drugs consumed in the hospital)of the 10 kinds of auxiliary drugs for clinical treatment decreased from 9.88%(from January to June 2020)to 7.05%(from July to December 2020).Conclusion Compared with the simple special comment intervention of adjuvant drugs,the use of adjuvant drugs in clinical departments tends to be more reasonable after the implementation of grid real-time monitoring by clinical pharmacists.
