摘要
目的探讨腹腔镜探查术在胃肠外科急诊治疗中的临床效果。方法选择2019年3月—2021年3月蚌埠医学院附属蚌埠市第三人民医院收治的行胃肠外科急诊治疗的80例患者进行研究,采用电脑盲选法分为对照组和研究组,每组40例,对照组用传统开腹手术,研究组采用腹腔镜探查术,对比两组手术时间、术后下床时间、手术出血量、住院时间、并发症率、生活质量测定简表(Quality of Life,QOL)评分。结果研究组手术时间(90.26±9.58)min、术后下床时间(0.62±0.18)d、手术出血量(32.41±5.22)mL以及住院时间(6.83±0.30)d明显优于对照组(178.45±12.17)min、(2.18±0.53)d、(63.09±8.71)mL、(12.54±0.69)d,差异有统计学意义(P<0.05);研究组并发症发生率7.50%明显低于对照组的25.00%,差异有统计学意义(χ^(2)=4.501,P<0.05);两组治疗前QOL评分差异无统计学意义(P>0.05),治疗后两组患者QOL评分均提高,且研究组明显高于对照组,差异有统计学意义(P<0.05)。结论腹腔镜探查术应用在胃肠外科急诊治疗中具有明显的治疗效果,减少手术、下床以及住院时间,还能够减少手术出血量、并发症发生率,提高患者生活质量,临床应用价值较高。
Objective To investigate the clinical effect of laparoscopic exploration in emergency treatment of gastrointestinal surgery.Methods A total of 80 patients admitted to Bengbu Third People's Hospital Affiliated to Bengbu Medical College from March 2019 to March 2021 who re⁃ceived emergency treatment for gastrointestinal surgery were selected for the study.They were divided into control group and study group by computer blind selection method.40 cases,the control group was treated with traditional laparotomy,and the study group was treated with laparoscopic exploration.The operation time,postoperative ambulation time,surgical blood loss,hospital stay,complication rate,and Qual⁃ity of Life(QOL)score were compared between the two groups.Results The operation time of the study group was(90.26±9.58)min,the time to get out of bed after surgery(0.62±0.18)d,the amount of surgical blood loss(32.41±5.22)mL and the length of hospital stay(6.83±0.30)d were significantly better than those of the control group(178.45±12.17)min,(2.18±0.53)d,(63.09±8.71)mL,and(12.54±0.69)d,the differences were statistically significant(P<0.05);the incidence of complications in the study group was 7.50%,which was significantly lower than that in the control group(25.00%),the difference was statistically significant(χ^(2)=4.501,P<0.05);there was no significant difference in the QOL scores between the two groups before treatment(P>0.05),and the QOL scores of the two groups were improved after treatment,and the study group was significantly higher than that in the control group,the difference was statistically significant(P<0.05).Conclusion The ap⁃plication of laparoscopic exploration in the emergency treatment of gastrointestinal surgery has obvious therapeutic effect,which can reduce the time of operation,getting out of bed and hospitalization,and can also reduce the amount of surgical blood loss,the incidence of complica⁃tions,and improve the quality of life of patients.The clinical application value is high.
