胎儿期冠状窦宽度与孕周的相关性分析

Correlation analysis between the width of coronary sinus and gestational weeks

摘要 目的 :测量21~40周胎儿冠状窦的宽度,观察冠状窦宽度与孕周的相关性及冠状窦宽度的正常值范围。方法:随机选取2019年1月至2020年7月来本院产检且孕龄≥21周的胎儿1 511例,测量冠状窦宽度。将入选的胎儿分为21~27周、28~35周、36周以上3组,测量每组的冠状窦宽度,并进行组间比较,对冠状窦宽度与孕周的相关性进行分析。结果 :随着孕周的增加,冠状窦宽度均有所增加,但冠状窦宽度与孕周无线性相关性;3组之间差异均有统计学意义;孕21~27周、28~35周和36周以上冠状窦宽度的95%的置信区间分别为0.88~2.95 mm、1.22~3.89mm和1.44~4.61mm。结论 :胎儿期冠状窦的宽度随着孕周的增加而有所增加,但与孕周无线性相关关系,当冠状窦的宽度超过其相应的95%的置信区间时,要结合临床考虑是否有相关疾病导致冠状窦宽度的增宽。 Objective : The width of the coronary sinus(CS)in the fetus at 21 to 40 weeks were measured to observe the correlation between the coronary sinus and the week of gestation and the normal range of the coronary sinus.Methods : 1 511 fetuses whose gestational age ≥ 21 weeks were randomly selected from January 2019 to July 2020in our hospital to measure the width of the coronary sinus. The enrolled fetuses were divided into three groups : 21to 27 weeks,28 to 35 weeks, and more than 36 weeks. The coronary sinus width of each group was measured and compared between groups to analyze the correlation between coronary sinus width and gestational weeks. Results : Coronary sinus width increased with increasing gestational weeks, but there was no linear correlation between coronary sinus width and gestational weeks. The differences among the three groups were statistically significant.The 95% confidence intervals for the internal diameters of coronary sinuses at 21 to 27 weeks of gestation, 28 to 35 weeks of gestation and more than 36 weeks of gestation were 0.88 to 2.95 mm, 1.22 to 3.89 mm and 1.44 to 4.61 mm, respectively. Conclusion : The width of the coronary venous sinus increases with gestational week of the fetal life, but there is no linear correlation with gestational week. When the width of the coronary sinus exceeds its corresponding 95% confidence interval, the possibility of widening of the coronary sinus due to an associated disease should be considered in the clinical context.
