OCTA与OCT分析原发性开角型青光眼视盘结构的比较 被引量:1

摘要 目的研究相干光层析血管成像术(OCTA)与相干光层析成像术(OCT)分析早期原发性开角型青光眼(POAG)视盘结构的相关性。方法横断面病例研究。收集2019年1月至2021年4月于福州东南眼科医院青光眼科门诊就诊患者,早期POAG组45例(70只眼),其中男性32例(49只眼),女性13例(21只眼),年龄(46.05±18.73)岁。OCTA检测视盘上方、下方、鼻侧、颞侧四个象限的血管密度,OCT检测视盘上方、下方、鼻侧、颞侧四个象限的视网膜神经纤维层厚度(RNFL),分析比较两者同个象限视盘结构改变的相关性。采用Shapiro-Wilk正态检验,不满足正态分布选择Spearman等级相关,满足正态分布选择Pearson线性相关检验进行统计分析。结果视盘RNFL上方、下方、颞侧分别与血管密度上方、下方、颞侧之间的相关关系显著性检验(P=0.013、0.000、P=0.030),差异有统计学意义,相关关系显著,相关系数分别为0.292、0.530、0.256,呈正相关,提示这三个象限的视盘血管密度随视盘RNFL变薄而减少;视盘RNFL(鼻侧)与血管密度(鼻侧)之间的相关关系显著性检验(P=0.110),无相关性;结论早期POAG的视盘血管密度和RNFL改变是一致,正相关的,特别是在视盘的上下方,视盘的血管密度随着RNFL的变薄而减少。通过OCTA测量视盘血管密度,可以为早期青光眼的临床诊断提供新的手段。 Objective To compare the correlation between optical coherence tomography(OCTA)and optical coherence tomography(OCT)in measuring optic disc of primary open angle glaucoma.Methods A cross-sectional study enrolled 70 eyes of 45 patients(49 eyes in 32 males and 21 eyes in 13 females)with age of(46.05±18.73)years who were diagnosed with early primary open angel glaucoma(POAG)at the department of glaucoma in Fuzhou Southeast Eye Hospital from January 2019 to April 2021.The optic vessel density was measured in superior、inferior、nasal and temporal area by OCTA,while the retinal nerve fiber layer(RNFL)was measured by OCT,exploring the relevance of the two examinations in the same area.Dates used Shapiro-Wilk normality test,Spearman’s rank correlation was chosen if dates did not conform to the normal distribution,the Pearson linear correlation test was chosen if dates conformed to the normal distribution.Results The correlation of RNFL and the optic vessel density was examined in the superaoptic、infraoptic and temporal regions(P=0.013,0.000,P=0.030),and it was statistically significance.The correlation coefficients also were significant,0.292,0.530,0.256 respectively,presenting positive correlation which suggested the optic vessel density deceased with RNFL thinned.The correlation of RNFL and the optic vessel density was tested in nasal area(P=0.110),difference was not significant,the correlation coefficient also was not significant.Conclusions The change of optic canal density of POAG in the early stage was consistent with that of retinal nerve fiber layer,and showed a positive correlation.It decreased with the thinning of retinal nerve fiber layer,especially in the upper and lower regions.OCTA detection of optic nerve vascular density is a new method in clinical diagnosis of early POAG.
