摘要
目的分析住院患者重点监控合理用药药品的不合理用药医嘱的帕累托图,为进一步合理使用重点监控药品提供依据。方法通过沈阳医学院附属中心医院住院系统电子病历平台选取2019年1月至2020年6月出院患者中使用重点监控药品的相关病历,用帕累托图法分析不合理用药医嘱的主要因素、次要因素及一般因素。结果通过对720份病历医嘱分析发现,5种不合理用药类型,主要因素有药物的用法用量不适宜、用药不规范、遴选药品不适宜;次要因素为适应证不适宜;一般因素为重复用药。结论通过对重点监控药品采取药事管理和合理用药干预,不合理用药问题得到了明显的改善。需进一步精细化对医师的合理用药培训内容,逐步提升医师合理用药水平,并且加强信息化支持对重点监控药品进行管控,促进重点监控药品的合理使用。
Objective To analyze of the irrational use of key monitoring drugs through Pareto chart among inpatients,and to provide basis and reference for further control of the irrational use of key controlled drugs.Methods Medical records related to the use of key monitored drugs in discharged patients from January 2019 to June 2020 were selected through the electronic medical record platform of the inpatient system of the Central Hospital Affiliated to Shenyang Medical College.The main factors,secondary factors and general factors of irrational drug use were analyzed through the Pareto chart method.Results In total,5 types of irrational drug use were found from 720 medical records.The primary factors of irrational drug use were improper usage and dosage and sub-standard medication and inappropriate selection of drugs.The secondary factor was inappropriate indications.The general factor was repeated drug use.Conclusion Through pharmaceutical administration and intervention measures of rational drug use,the irrational drug use has been significantly improved,It is necessary to further refine the training content of rational drug use for physicians,gradually improve the level of rational drug use,and strengthen information support for the control of key monitoring drugs,which promote the reasonable use of key monitoring drugs.
