摘要
目的:应用Meta分析评价中药辅助灌肠对肝性脑病(Hepatic Encephalopathy,HE)的影响。方法:检索自建库至2020年9月知网、维普、万方、CBM、中国临床试验中心、EMbase、Cochrane Library、PubMed、Web of Science中治疗组为中药灌肠联合西医常规治疗,对照组为乳果糖灌肠联合西医常规治疗,关于肝性脑病的临床随机对照试验;制定纳入与排除标准,遴选文献,录取数据,使用RevMan5.3软件进行Meta分析。结果:共纳入5篇文献,330例患者。Meta分析结果显示:治疗组血氨值[均数差(Mean Difference,MD)=-4.11,95%置信区间(Confidence Interval,CI)=(-5.65,-2.58),P<0.00001]、总胆红素[MD=-5.23,95%CI=(-8.67,-1.79),P=0.003<0.05]、丙氨酸氨基转移酶[MD=-5.57,95%CI=(-9.71,-1.43),P=0.008<0.05]均优于对照组,两组总有效率[相对危险度(Risk Ratio,RR)=1.12,95%CI=(0.98,1.26),P=0.09>0.05]基本相同。结论:中药辅助灌肠在降低肝性脑病患者的血氨、总胆红素及丙氨酸氨基转移酶上优于乳果糖灌肠。
Objective:To evaluate the effect of enema with traditional Chinese medicine on hepatic encephalopathy by Metaanalysis.Methods:CNKI,VIP,Wanfang,CBM,China Clinical Trial Center,EMbase,Cochrane Library,PubMed,Web of Science from the construction of the database to September 2020 were searched.The treatment group received traditional Chinese medicine enema combined with conventional Western medicine treatment,and the control group received lactulose enema combined with conventional Western medicine treatment.Clinical randomized controlled trials were on HE;Inclusion and exclusion criteria were formulated,literature selection and admission data were selected,and Meta-analysis was conducted using RevMan5.3 software.Results:A total of 5 articles and 330 patients were included.Meta analysis results show blood ammonia value[MD=-4.11,95%CI=(-5.65,-2.58),P<0.00001],total bilirubin[MD=-5.23,95%CI=(-8.67,-1.79)),P=0.003<0.05],alanine aminotransferase[MD=-5.57,95%CI=(-9.71,-1.43),P=0.008<0.05],total effective rate[RR=1.12,95%CI=(0.98,1.26),P=0.09>0.05].Conclusion:Chinese medicine enema is better than lactulose enema in reducing blood ammonia,total bilirubin and alanine aminotransferase in patients with hepatic encephalopathy.
