摘要
目的观察雷火灸联合心理疗法对气滞血瘀型混合痔术后肛门疼痛及创面愈合的影响。方法将90例气滞血瘀型混合痔术后患者按照随机数字表法分为3组,治疗组30例予雷火灸联合心理疗法治疗,对照组30例予雷火灸法治疗,止痛药组30例予口服氨酚双氢可待因片治疗,3组均治疗6 d。比较3组术后3、5、7 d疼痛视觉模拟评分(VAS)、水肿评分及术后14、20 d创面愈合率。结果3组术后5、7 d疼痛VAS均较本组术后3 d降低(P<0.05),3组术后7 d均低于本组术后5 d(P<0.05),且治疗组术后3、5、7 d疼痛VAS均低于对照组、止痛药组同期(P<0.05),止痛药组术后3、5、7 d疼痛VAS均低于对照组同期(P<0.05)。治疗组、对照组术后5 d及3组术后7 d水肿评分均较本组术后3 d降低(P<0.05),3组术后7 d均低于本组治疗5 d(P<0.05),且治疗组术后3、5、7 d水肿评分均低于对照组、止痛药组同期(P<0.05),对照组术后3、5、7 d水肿评分均低于止痛药组同期(P<0.05)。治疗组、对照组术后14、20 d创面愈合率均高于止痛药组(P<0.05)。结论雷火灸联合心理疗法对缓解气滞血瘀型混合痔术后肛门疼痛、水肿及促进创面愈合疗效显著,操作简便易行且安全性高,有着良好的发展前景。
Objective To explore the clinical effect of thunder-fire moxibustion combined with psychotherapy on postoperative anal pain and wound healing of mixed hemorrhoids with qi stagnation and blood stasis.Methods Totally 90 postoperative patients with mixed hemorrhoids were randomly divided into three groups,30 cases in each,the treatment group was treated with thunder-fire moxibustion combined with psychotherapy,the control group and the analgesic group with thunder-fire moxibustion or oral Aminophenol dihydrocodeine tablets.All three groups were treated for 6 days.The visual analogue scale(VAS),the edema score and wound healing rate on days 14 and 20 of the postoperation were compared among the three groups.Results In the 3 groups,VAS on days 5 and 7 was lower than that on day 3(P<0.05),VAS on days 7 was lower than that on day 5(P<0.05).VAS in treatment group was the lowest among the 3 groups,following the control group and the analgesic group(all P<0.05).The edema scores on days 5 and 7 was lower than that on day 3(P<0.05),which on days 7 was lower than that on day 5(P<0.05).The edema scores in treatment group was the lowest among the 3 groups,following the control group and the analgesic group(all P<0.05).After operation,the wound healing rates on days 14 and 20 in the treatment group and the control group were higher than those in the analgesic group(P<0.05).Conclusion For patient with mixed hemorrhoids(qi stagnation and blood stasis),thunder-fire moxibustion combined with psychotherapy has significant curative effect on postoperative anal pain and wound healing,the method is easy and feasible,with high safety and a good development prospect.
