摘要
目的探讨早期生化检验对糖尿病肾病的诊断效果与血清学指标的影响。方法本研究选取山东省曲阜市中医院于2020年1月至2021年1月收治的100例糖尿病肾病患者(试验组)和100名健康体检者(对照组)作为研究对象,对其进行早期生化检验,并分析检验结果,观察并分析两组患者的生化检验指标,包括尿微量白蛋白(mAlb)、尿糖(GLU)、超敏C反应蛋白(hs-CRP)、血肌酐(Cr)、血清胱抑素C(CysC)、糖化血红蛋白(HbA_(1)c)等。观察两组生化检验的诊断效能,包括联合检验和单一检验的准确度、特异度和灵敏度等。结果试验组mAlb、GLU明显比对照组高,差异有统计学意义(P<0.05)。试验组hs-CRP、Cr、CysC、HbA_(1)c明显比对照组高,差异有统计学意义(P<0.05)。尿液生化检验联合血液生化检验联合准确度(99.00%)、特异度(85.00%)和灵敏度(90.00%)均比单一检验(尿液生活检验:81.00%、72.00%、75.00%;血液生化检验:83.00%、70.00%、73.00%)更高,差异有统计学意义(P<0.05)。结论临床在诊断糖尿病肾病患者时,予以早期生化检验,通过分析mAlb、GLU、Cr、HbA_(1)c、CysC、hs-CRP水平变化能够对糖尿病肾病进行诊断,但单一尿液或者血液生化检验的准确度、特异度、灵敏度欠佳,建议选择尿液生化检验联合血液生化检验的方式,以使整体诊断效能得以提高。
Objective To investigate the effect of early biochemical examination on the diagnosis of diabetic nephropathy and serum level.Methods A total of 100 patients with diabetic nephropathy(experimental group)and 100 healthy persons(control group)from January 2020 to January 2021 in Qufu Hospital of Traditional Chinese Medicine in Shandong Province were selected as the research subjects.They were given early biochemical tests,and the results of the analysis were carefully analyzed.The biochemical indexes of the two groups were observed and analyzed,including mAlb,GLU,hs-CRP,Cr,CysC,HbA_(1)c,etc.The diagnostic efficacy of biochemical test was observed and analyzed,including the accuracy,specificity and sensitivity of combined test and single test.Results The levels of urine biochemical test indexes(mAlB,GLU)in the experimental group were significantly higher than those in the control group(P<0.05).The levels of blood biochemical test indexes(hsCRP,Cr,CysC,HbA_(1)c)in the experimental group were significantly higher than those in the control group(P<0.05).The accuracy,specificity and sensitivity of urine biochemical test combined with blood biochemical test were 99.00%,85.00%and 90.00%,which were higher than those of single test(urine life test:81.00%,72.00%,75.00%;blood biochemical test:83.00%,70.00%,73.00%),the differences were statistically significant(P<0.05).Conclusion In the clinical diagnosis of diabetic nephropathy,early biochemical tests are performed.By analyzing mAlb,GLU,Cr,HbA_(1)c,CysC,hs-CRP and other related indicators,we can compare with the normal range of levels,combined with clinical manifestations,symptoms and so on,can accurately diagnose diabetic nephropathy.However,single urine biochemical tests or blood biochemical tests,the accuracy,specificity,sensitivity and other effects are not good,we should select urine biochemical test combined with blood biochemical examination mode,so as to improve the overall diagnostic efficiency.
