摘要
目的:探讨经阴道二维超声检测孕早期黄体血流参数与妊娠早期结局的关系。方法:收集2016年1月-2021年7月在某院就诊的早孕受试者临床资料。根据受试者孕12周的妊娠结局,将受试者分为正常妊娠组(n=256)和妊娠终止组(n=44),比较两组受试者黄体血流超声参数差异,使用ROC曲线分别探索不同超声参数与妊娠终止的关系。结果:两组受试者年龄、停经时间、孕次、吸酒史、BMI之间的差异均无统计学意义(P>0.05)。正常妊娠组患者黄体D值更大(P=0.000)、RI值(P=0.022)和PI值(P=0.000)更低。孕早期黄体超声参数RI预测受试者妊娠12周时妊娠终止的ROC曲线下面积最大为0.954,最佳诊断切点为14.51,此时敏感度、特异度分别为98.6%、68.4%。结论:孕早期经阴道超声黄体血流参数对孕12周妊娠结局具有较高预测价值,值得今后进一步研究。
Objective:To investigate the relationship between luteal blood flow parameters detected by transvaginal two-dimensional ultrasound and early pregnancy outcome.Methods:The clinical data of early pregnancy subjects treated in a hospital from January 2017 to July 2021 were collected.According to the pregnancy outcome at 12 weeks of pregnancy,the subjects were divided into normal pregnancy group(n=256)and pregnancy termination group(n=44).The differences of luteal blood flow ultrasonic parameters between the two groups were compared,and the relationship between different ultrasonic parameters and pregnancy termination was explored by ROC curve.Results:There was no significant difference between the two groups in age,menopause time,pregnancy times,alcohol smoking history and BMI(P>0.05).The luteal D value(P=0.000),RI value(P=0.022)and PI value(P=0.000)were lower in the normal pregnancy group.The maximum area under the ROC curve of luteal ultrasound parameter RI in early pregnancy in predicting termination pregnancy at 12 weeks of pregnancy was 0.954,and the best diagnostic cut-off point was 14.51.At this time,the sensitivity and specificity were 98.6%and 68.4%,respectively.Conclusion:Transvaginal ultrasound luteal blood flow parameters in early pregnancy have high predictive value for pregnancy outcome at 12 weeks of pregnancy,which is worthy of further study in the future.
