摘要
目的:观察通调三焦法对慢性胃炎伴幽门螺杆菌(helicobactor pylori,Hp)阳性患者Hp清除率及中医证候评分的影响。方法:选取2020年10月至2021年10月大连大学附属新华医院中医科收治的慢性胃炎伴幽门螺杆菌阳性患者80例,随机分为对照组和试验组,每组40例。对照组根据患者基本病情给予西医规范化四联疗法治疗,试验组在对照组治疗的基础上加用金宇安通调三焦法治疗。观察两组患者治疗前后急性生理与慢性健康量表Ⅱ评分、胃肠功能障碍评分、序贯器官衰竭评估(sequential organ failure assessment,SOFA)、肠道功能评分量表(the gastrointestinal failure score,GIF)、中医证候积分、炎症指标、Hp清除率、免疫功能各项指标情况。结果:试验组有效率为95.00%,对照组有效率为85.00%,试验组有效率高于对照组,差异有统计学意义(P<0.05);两组治疗后急性生理与慢性健康量表Ⅱ评分、胃肠功能障碍评分、SOFA评分、GIF评分低于治疗前,且试验组治疗后急性生理与慢性健康量表Ⅱ评分、胃肠功能障碍评分、SOFA评分、GIF评分低于对照组,差异有统计学意义(P<0.05);两组治疗后WBC、PCT、CRP水平低于治疗前,且试验组低于对照组,差异有统计学意义(P<0.05);两组治疗后中医证候积分低于治疗前,且试验组治疗后中医证候积分低于对照组,差异有统计学意义(P<0.05);试验组Hp清除率为92.50%,对照组Hp清除率为75.00%,试验组Hp清除率高于对照组,差异有统计学意义(P<0.05);试验组治疗后免疫功能水平优于对照组,差异有统计学意义(P<0.05)。结论:通调三焦法治疗慢性胃炎伴幽门螺杆菌阳性疗效显著,有效清除Hp,改善患者中医证候、肠道屏障功能损伤状况及免疫功能。
Objective: To observe the influence method of dredging triple energizer on Hp clearance rate and TCM syndrome score in patients with chronic gastritis and Hp positive.Methods: A total of 80 patients with chronic gastritis and Hp positive admitted to the Department of Traditional Chinese Medicine, Dalian University Affiliated Xinhua Hospital from October 2020 to October 2021 were randomly divided into the control group and the experimental group, with 40 cases in each group.The control group was treated with western medicine standardized quadruple therapy according to the basic condition of the patients, while the experimental group was treated with Jin Yu′an′s method of dredging triple energizer on the basis of the treatment of the control group.The acute physiology and chronic health scale II score, gastrointestinal dysfunction score, sequential organ failure assessment(SOFA),the gastrointestinal failure score(GIF),TCM syndrome score, inflammation index, Hp clearance rate and immune function were observed before and after treatment between the two groups.Results: The effective rate of the experimental group was 95.00%,and that of the control group was 85.00%.The effective rate of the test group was significantly higher than that of the control group, and the difference was statistically significant(P<0.05).After treatment, the scores of acute physiology and chronic health scale II,gastrointestinal dysfunction, SOFA and GIF in the two groups were lower than those before treatment, and the scores of acute physiology and chronic health scale II,gastrointestinal dysfunction, sequential organ failure and GIF in the experimental group were significantly lower than those in the control group, and all the differences were statistically significant(P<0.05).The levels of WBC,PCT and CRP in the two groups after treatment were lower than those before treatment, and the above indexes in the experimental group were lower than those in the control group, and all the differences were statistically significant(P<0.05).The TCM syndrome score of the two groups after treatment was lower than that before treatment, and the TCM syndrome score of the experimental group after treatment was lower than that of the control group, and all the differences were statistically significant(P<0.05).The Hp clearance rate was 92.50% in the experimental group, which was significantly higher than 75.00% in the control group, and the difference was statistically significant(P<0.05).The level of immune function in the experimental group was better than that in the control group after treatment, and the difference was statistically significant(P<0.05).Conclusion: Method of dredging triple energizer has a significant effect in the treatment of chronic gastritis with Hp positive, can effectively clear Hp, improve patients′ TCM syndromes, and intestinal barrier function damage and immune function.
