摘要
目的分析帆状胎盘孕产妇分娩方式及围产儿结局。方法以我院2019年6月至2020年11月收治的226例帆状胎盘孕产妇为观察组,再以同期300例健康孕产妇为对照组。比较两组的临床特点、分娩方式;比较两组的围产儿结局及孕产妇产后抑郁发生率;分析观察组围产儿死亡原因。结果观察组三次妊娠、双胎及三胎妊娠、剖宫产占比均显著高于对照组(P<0.05);观察组的各项合并症发生率均显著高于对照组(P<0.05)。观察组的胎盘早剥、胎儿宫内窘迫、早产、低体重儿、死胎、死产及产后抑郁发生率均显著高于对照组(P<0.05)。观察组围产儿死亡原因中,低体重儿占比最高(40.43%)。结论临床应重视产前诊断以提高帆状胎盘早期检出率,并适时终止妊娠,降低不良妊娠结局发生率。
Objective To analyze the mode of delivery of pregnant and lying-in woman with velamentous placenta and perinatal outcomes.Methods A total of 226 pregnant and lying-in woman with velamentous placenta admitted in our hospital from June 2019 to November 2020 were selected as observation group,and 300 healthy pregnant and lying-in woman in the same period were selected as control group.The clinical characteristics and mode of delivery of the two groups were compared;the perinatal outcomes and incidence of postpartum depression of pregnant and lying-in woman were compared between the two groups;the cause of perinatal death in the observation group was analyzed.Results The proportions of third pregnancy,twin and tripte pregnancy and cesarean section in the observation group were significantly higher than those in the control group(P<0.05);the incidences of complications in the observation group were significantly higher than those in the control group(P<0.05).The incidences of placental abruption,fetal distress,premature delivery,low birth weight,stillborn foetus,stillbirth and postpartum depression in the observation group were significantly higher than those in the control group(P<0.05).Among the perinatal death causes in the observation group,low birth weight accounted for the highest proportion(40.43%).Conclusion Clinical attention should be paid to prenatal diagnosis in order to improve the early detection rate of velamentous placenta,terminate pregnancy in time and reduce the incidence of adverse pregnancy outcomes.
