摘要
目的探讨心力衰竭患者血清短链脂肪酸(SCFA)、氧化三甲胺(TMAO)表达与肠道菌群及代谢综合征(MS)的相关性。方法选取2020年1月至2020年10月在本院就诊的100例心力衰竭患者作为观察组,另外选取100例同期健康体检者作为对照组。比较两组收缩压、舒张压、空腹血糖(FBG)、低密度脂蛋白胆固醇(LDL-C)、高密度脂蛋白胆固醇(HDL-C)、总胆固醇(TC)、三酰甘油(TG)、肠道菌群以及血清SCFA、TMAO表达水平差异;采用Spearman分析观察组血清SCFA、TMAO表达水平与代谢相关参数、肠道菌群指标相关性。结果与对照组相比,观察组舒张压、HDL-C、TC及变形菌门、放线菌门、梭杆菌门丰度差异均无统计学意义(P>0.05),收缩压、FBG、LDL-C、TG、厚壁菌门丰度及TMAO表达水平均较高(P<0.05),拟杆菌门丰度及血清乙酸、丙酸、丁酸表达水平较低(P<0.05)。Spearman分析结果显示,观察组血清乙酸、丙酸、丁酸表达水平与收缩压、TG、LDL-C、FBG、厚壁菌门均呈负相关(P<0.05),与拟杆菌门呈正相关(P<0.05);TMAO与收缩压、TG、LDL-C、FBG、厚壁菌门均呈正相关(P<0.05),与拟杆菌门呈负相关(P<0.05)。结论心力衰竭患者血清乙酸、丙酸、丁酸为低表达,TMAO为高表达,均与肠道菌群及MS存在一定相关性,调控机体肠道菌群代谢产物SCFA、TMAO可能成为治疗心力衰竭的新思路。
Objective To investigate the correlation of serum short chain fatty acid(SCFA)and trimethylamine oxide(TMAO)with intestinal flora and metabolic syndrome(MS)in patients with heart failure.Methods From January,2020 to October,2020,100 patients with heart failure admitted in our hospital were selected as the observation group,while 100 healthy people during the same period were selected as the control group.Systolic blood pressure(SBP),diastolic blood pressure(DBP),fasting blood glucose(FBG),low density lipoprotein cholesterol(LDL-C),high density lipoprotein cholesterol(HDL-C),total cholesterol(TC),triglyceride(TG),intestinal flora,SCFA and TMAO expression levels in patients’serum were compared between the two groups;Spearman analysis was applied to analyze the correlation between serum SCFA,TMAO expression levels and metabolism related parameters,intestinal flora indicators in the observation group.Results Compared with those in the control group,there were no significant differences in diastolic blood pressure,HDL-C,TC and abundances of Proteus,actinomycetes and fusobacteria in the observation group(P>0.05),while systolic blood pressure,FBG,LDL-C,TG,firmicum abundance and TMAO expression levels were higher(P<0.05),and bacteroides abundance and serum levels of acetic acid,propionic acid and butyric acid were lower(P<0.05).Spearman analysis showed that the expression levels of serum acetic acid,propionic acid and butyric acid in the observation group were negatively correlated with systolic blood pressure,TG,LDL-C,FBG and firmicum(P<0.05),while positively correlated with Bacteroides(P<0.05);TMAO was positively correlated with systolic blood pressure,TG,LDL-C,FBG and firmicum(P<0.05),but negatively correlated with Bacteroides(P<0.05).Conclusion Serum levels of acetic acid,propionic acid and butyric acid are low and the level of TMAO is high in patients with heart failure,and there is a significant correlation between these markers and intestinal flora,MS.Regulating the metabolites of intestinal flora SCFA and TMAO may be a new approach to treat heart failure.
