摘要
目的:探讨超声引导下微波消融术治疗乳腺良性结节的临床价值。方法:回顾性分析2018年1月-2020年10月赣州市人民医院收治的98例乳腺良性结节手术患者的临床资料,根据患者手术方式差异分为开放手术组(34例,接受传统开放手术治疗)、微创旋切组(32例,接受麦默通微创手术治疗)、微波消融组(32例,接受超声引导下微波消融术治疗),所有患者术后均随访12个月。比较三组视觉模拟评分法(VAS)评分、肿瘤坏死因子-α(TNF-α)、C反应蛋白(CRP)、瘢痕美容评估与评级(SCAR)、疗效和并发症发生情况。结果:术后1、2、3、24、48 h开放手术组的VAS评分及术后7 d的TNF-α、IL-6水平均高于微创旋切组与微波消融组,差异均有统计学意义(P<0.05)。微创旋切组与微波消融组术后1、2、3、24、48 h的VAS评分及术后7 d的TNF-α、IL-6水平比较,差异均无统计学意义(P>0.05)。术后12个月,微波消融组的瘢痕优良率高于开放手术组,并发症发生率低于开放手术组,差异均有统计学意义(P<0.05)。术后12个月,微波消融组与开放手术组的总有效率均高于微创旋切组,差异均有统计学意义(P<0.05)。结论:超声引导下微波消融术治疗乳腺良性结节的疗效良好,并发症少、疼痛感轻、手术感染风险低且操作简单。
Objective:To explore the clinical value of ultrasound-guided microwave ablation in the treatment of benign breast nodules.Method:The clinical data of 98 patients with benign breast nodules in Ganzhou People’s Hospital from January 2018 to October 2020 were retrospectively analyzed,according to the difference of operative methods,patients were divided into open operation group(34 cases,received traditional open operation),minimally invasive rotary resection group(32 cases,received Maimerton minimally invasive operation)and microwave ablation group(32 cases,received ultrasound-guided microwave ablation).All patients were followed up for 12 months after operation.Visual analogue scale(VAS)score,tumor necrosis factor-α(TNF-α),C reactive protein(CRP),scar cosmesis assessment and rating(SCAR),efficacy and complications were compared among three groups.Result:The VAS scores at 1,2,3,24 and 48 h after operation,TNF-αand IL-6 levels at 7 d after operation in the open operation group were higher than those in the minimally invasive rotary resection group and microwave ablation group,the differences were statistically significant(P<0.05).There were no significant difference in VAS scores at 1,2,3,24 and 48 h after operation,TNF-αand IL-6 levels at 7 d after operation between the minimally invasive rotary resection group and microwave ablation group,the differences were statistically significant(P>0.05).12 months after operation,the excellent rate of scar in the microwave ablation group was higher than that in the open operation group,and the incidence of complications in the microwave ablation group was lower than that in the open operation group,the differences were statistically significant(P<0.05).12 months after operation,the total effective rates in the microwave ablation group and open operation group were higher than those in the minimally invasive rotary resection group,the differences were statistically significant(P<0.05).Conclusion:Ultrasound-guided microwave ablation is effective in the treatment of benign breast nodules,with less complications,less pain,low risk of surgical infection and simple operation.
