摘要
目的 通过考察武汉市三级甲等公立综合医院急诊科医生工作偏好情况,为稳定和吸引急诊卫生人力资源提供依据。方法 采用分层整群抽样方法,对武汉市8家三级甲等公立综合医院225名急诊科医生进行线上离散选择实验。结果 (1)每周夜班数量、月收入增幅、职称晋升要求、执业风险程度、聘用类型对急诊科医生工作选择偏好的影响均有统计学意义(P<0.05);(2)非货币属性中,急诊科医生首先考虑的是每周夜班数量,其次依次为职称晋升要求、聘用类型、执业风险程度;(3)相对无离职意愿的急诊科医生存在潜在离职意愿的医生群体对月收入存在明显偏好。结论 可通过科学设计急诊科医生薪酬绩效制度,优化夜班轮班模式,强制性执行夜班间歇性休息制度以改善急诊医生工作环境。在此基础上,合理运用职称制度杠杆,科学调整科研、业务评定权重结构,逐步推进去编制化改革。
Objective To provide a basis for stabilizing and attracting emergency health human resources by investigating the work preference of emergency doctors in Wuhan Class-A tertiary public general hospitals.Methods Using the stratified cluster sampling method,225 emergency doctors(EDs)in 8 Class-A tertiary public general hospitals in Wuhan were investigated online using a discrete choice experiment of job preference.Results(1)The number of night shifts per week,monthly income increase,requirements for promotion of professional titles,degree of practice risk,and employment type had statistically significant effects on EDs’job preferences(P<0.05);(2)among non-monetary attributes,EDs first considered the number of night shifts per week,followed by title promotion requirements,type of employment,and practice risk degree;(3)compared with emergency physicians who have no intention to leave,the group of doctors with potential intention to leave has a clear preference for monthly income.Conclusion The salary performance system for EDs should be scientifically designed,the night shift mode could be optimized,and the night shift intermittent rest system should be enforced to improve the working environment of EDs.On this basis,rationally using the levers of the professional title system,scientifically adjusting the weight structure of scientific research and business evaluation,and gradually promoting the authorized strength disappearing reform.
