摘要
目的:探讨穴位贴敷联合中药热奄包治疗慢性阻塞性肺疾病(COPD)并发便秘的临床效果。方法:2018年1月—2019年12月该院收治的64例COPD合并便秘患者,采用随机数字表法将其分成观察组(n=32)及对照组(n=32)。对照组给予常规治疗,观察组在此基础上采用穴位敷贴联合中药热奄包治疗,所有患者均治疗2周。比较两组临床疗效,治疗前后便秘相关症状评分、便秘患者生活质量自评量表(PAC-QOL)评分、肠道菌群数量变化,副作用发生情况。结果:观察组总有效率为96.88%(31/32),与对照组的68.75%(22/32)相比显著升高(P<0.05)。两组治疗后各项便秘症状(排便困难、粪便性状、排便时间等)评分均较治疗前显著降低(P<0.05),且观察组下降更显著(P<0.05)。与治疗前对比,两组治疗后PAC-QOL中各维度(躯体不适、心理社会不适、担心和焦虑、满意度)评分及其总分均显著减少(P<0.05);而观察组降低亦更显著(P<0.05)。两组治疗后肠道双歧杆菌、乳酸杆菌数量较治疗前均显著增加(P<0.05),肠杆菌、肠球菌数量均显著减少(P<0.05);且观察组改善更显著(P<0.05)。两组中仅观察组发生2例皮肤轻微红肿。结论:穴位贴敷联合中药热奄包治疗能显著缓解COPD并发便秘患者的便秘症状,维持肠道微生态平衡,提高生活质量,疗效确切,且患者耐受性较好。
Objective:To explore the clinical effect of acupoint sticking combined with traditional Chinese medicine Reyanbao in the treatment of chronic obstructive pulmonary disease (COPD) complicated by constipation.Methods:From January 2018 to December 2019,64 patients with COPD and constipation admitted to our hospital were divided into observation group (n=32) and control group (n=32) by random number table method.The control group was given routine treatment,and the observation group was given acupoint application combined with traditional Chinese medicine Reambao on this basis.All patients were treated for 2 weeks.The clinical efficacy,constipation-related symptom score,constipation patient quality of life self-rating scale (PAC-QOL) score,changes in the number of intestinal flora,and the occurrence of side effects before and after treatment were compared between the two groups.Results:The total effective rate of the observation group was 96.88% (31/32),which was significantly higher than that of the control group 68.75% (22/32) (P<0.05).The scores of various constipation symptoms (difficulty in defecation,fecal character,defecation time,etc.) after treatment in the two groups were significantly lower than those before treatment (P<0.05),and the decrease in the observation group was more significant (P<0.05).Compared with before treatment,the scores of each dimension (physical discomfort,psychosocial discomfort,worry and anxiety,satisfaction) in PAC-QOL and their total scores were significantly decreased in the two groups after treatment (P<0.05);the decrease in the observation group was also greater significant (P<0.05).After treatment,the numbers of intestinal bifidobacteria and lactobacilli in the two groups were significantly increased compared with those before treatment (P<0.05),while the numbers of enterobacteria and enterococci were significantly decreased (P<0.05);and the improvement in the observation group was more significant (P<0.05).In the two groups,only 2 cases of mild redness and swelling occurred in the observation group.Conclusion:Acupoint sticking combined with traditional Chinese medicine Reambao can significantly relieve constipation symptoms in patients with COPD complicated by constipation,maintain intestinal microecological balance,and improve quality of life,with definite curative effect and good patient tolerance.
