摘要
目的 分析张家口地区浅部真菌病病原菌分布、感染因素及药物敏感性,为本地区的浅部真菌感染防治提供临床依据。方法 选择2019年4月-2021年4月来本院皮肤科门诊就诊的521例浅部真菌病患者作为研究对象,200例非感染患者作为对照组;所有取样行镜检,并对病原菌进行鉴别且行药物敏感性检测。结果 张家口地区浅部真菌病病原菌中红色毛癣菌占比最高,在所有浅部感染真菌患者中以体癣占比最高;患者年龄、发病季节、外伤、长期服用抗生素、长期接触宠物及不良生活习惯是影响浅部感染真菌发生发展的独立性威胁因素,且存在统计学意义(P<0.05);特比萘芬与酮康唑联合应用对受试菌的抑制MIC值最低,其次分别为特比萘芬、萘替芬联合酮康唑、单用萘替芬及单用酮康唑。结论 张家口地区浅部真菌病病原菌以红色毛癣菌占比最高,年龄、发病季节、外伤、长期服用抗生素、长期接触宠物及不良生活习惯是重要的影响感染的因素,酮康唑与特比萘芬联合应用具有较高的抗真菌敏感性。
Objective To analyze the pathogen distribution, infection risk factors and drug sensitivity of superficial mycosis in the Zhangjiakou area to provide clinical evidence for prevention and treatment of superficial fungal infection in this area.Methods A total of 521 patients with superficial mycosis attending the Dermatology Clinic of our hospital from April 2019 to April 2021 were selected as the research subjects, and 200 non-infected patients as the control group.All samples were subjected to microscopic examination, pathogen identification and drug sensitivity test.Results Trichophyton rubrum accounted for the highest proportion of the pathogens of superficial mycosis in the Zhangjiakou area, and tinea corporis accounted for the highest proportion among all superficial fungal infections.Patients′ age, onset season, trauma, long-term use of antibiotics, long-term contact with pets and bad living habits were significant independent risk factors affecting the occurrence and development of superficial fungal infection.Terbinafine combined with ketoconazole had the lowest minimum inhibitory concentration, followed by terbinafine, naftifine combined with ketoconazole, naftifine alone and ketoconazole alone.Conclusion Trichophyton rubrum accounts for the highest proportion of superficial mycosis in Zhangjiakou.Age, onset season, trauma, long-term use of antibiotics, contact with pets and bad living habits affect infection.The combination of ketoconazole and terbinafine has high antifungal sensitivity.
