摘要
目的:探讨两种不同方法对牙周牙髓联合病变患者牙周症状、氧化应激反应的影响。方法:选取2019年1月-2021年6月在上海市第一康复医院口腔科治疗的94例牙周牙髓联合病变患者,采用随机数字表法将患者分为对照组和研究组,每组47例(47颗患牙)。对照组给予根管治疗术+术后替硝唑口服,研究组则在对照组的基础上加用盐酸米诺环素软膏治疗。比较两组疗效、牙周指标、应激指标及不良反应发生情况。结果:研究组总有效率高于对照组,差异有统计学意义(P<0.05)。治疗后,研究组牙周探诊深度(PD)、牙龈指数(GI)、龈沟出血指数(SBI)、菌斑指数(PLI)均低于对照组,差异均有统计学意义(P<0.05)。治疗后,两组丙二醛(MDA)、超氧化物歧化酶(SOD)水平均优于治疗前,且研究组均优于对照组,差异均有统计学意义(P<0.05)。两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论:米诺环素联合根管治疗术在牙周牙髓联合病变患者中应用效果让人满意,可有效提高临床治疗效果,改善患者牙周症状,降低患者的氧化应激反应,且不增加不良反应,是一种安全、有效的治疗方案,值得临床推广应用。
Objective:To investigate the effects of two different methods on periodontal symptoms and oxidative stress in patients with combined periodontal and pulpal lesions.Method:A total of 94 patients with combined periodontal and pulpal lesions treated in the Department of Stomatology of Shanghai First Rehabilitation Hospital from January 2019 to June 2021 were selected,they were divided into control group and study group according to random number table method,with 47 patients(47 affected teeth)in each group.The control group was given root canal therapy+postoperative oral Tinidazole,and the study group was given Minocycline Hydrochloride Ointment on the basis of the control group.The curative effect,periodontal index,stress index and the incidence of adverse reactions were compared between two groups.Result:The total effective rate in the study group was higher than that in the control group,the difference was statistically significant(P<0.05).After treatment,the periodontal probing depth(PD),gingival index(GI),gingival sulcus bleeding index(SBI)and plaque index(PLI)in the study group were lower than those in the control group,the differences were statistically significant(P<0.05).After treatment,the levels of malondialdehyde(MDA)and superoxide dismutase(SOD)in both groups were better than those before treatment,and those in the study group were better than those in the control group,the differences were statistically significant(P<0.05).There was no significant difference in the incidence of adverse reactions between two groups(P>0.05).Conclusion:Minocycline combined with root canal therapy has satisfactory application effect in patients with combined periodontal and pulpal lesions,it can effectively improve the clinical curative effect,improve the periodontal symptoms,reduce the oxidative stress of the patients,and does not increase the adverse effects,it is a safe and effective treatment plan,which is worthy of clinical application.
