摘要
徐小云教授认为,荨麻疹病机为皮肤腠理开阖失常,或卫气功能失常,导致邪气反复侵扰皮肤,治疗应抗御邪气,驱邪外出。其基于“其在皮者,汗而发之”理论,运用汗法开宣腠理,驱邪外出,并在此基础上,健运脾胃,充实卫气,增强皮肤抗邪能力,防止邪气再入侵。基础方组成为羌活、荆芥、防风、紫苏叶、白芷、桑叶、薄荷、浮萍中的2~3味药物,辛开腠理,驱邪外出;炒白术、茯苓、甘草、六神曲、陈皮健运脾胃;川芎、厚朴行气活血,通行营卫;炒白芍益阴敛营固表;白鲜皮、地肤子祛风止痒。并根据辨证实际,选择泻下、和解、清热、温里、补益、理气、理血、治风、治燥、祛湿、祛痰、消食、驱虫等治法。
Professor Xu Xiao-yun believes that the pathogenesis of urticaria is the abnormal opening and closing of the skin interstitial striae,or the dysfunction of defensive qi,which leads to the repeated intrusion of pathogenic qi into the skin.Therefore it is necessary to resist the pathogenic qi and drive it out.Based on the theory that "If pathogenic factors are in the skin,sweating therapy can be used to disperse",sweating therapy can be used to open skin interstitial striae and drive pathogens out.Besides,invigorating the spleen and stomach,enriching defensive qi,enhancing the ability to resist the skin pathogens and prevent the re-invasion of the pathogenic qi can be added.The basic formula is composed of 2-3 drugs from Qianghuo(Rhizoma et Badix Notopteryqii),Jingjie(Herba Schizonepetae),Fangfeng(Radix Saposhnikoviae),Zisuye(Folium Perillae),Baizhi(Radix Angelicae Dahuricae),Sangye(Folium Mori),Bohe(Herba Menthae) and Fuping(Herba Spirodelae),which are pungent to open skin interstitial striae,and drive pathogens out.Stir-fried Baizhu(Rhizoma Atractylodis Macrocephalae),Fuling(Poria),Gancao(Radix Glycyrrhizae),Liushenqu(Massa Fermentata Medicinalis) and Chenpi(Pericarpium Citri Reticulatae) can be used to invigorate the spleen and stomach.Chuanxiong(Rhizoma Chuanxiong) and Houpo(Cortex Magnoliae Officinalis) can be used to move qi and promote blood circulation and drive nutrient and defense qi.Stir-fried Baishao(Radix Paeoniae Alba) can replenish yin,condense nutrient qi and consolidate the superficies.Baixianpi(Cortex Dictamni) and Difuzi(Fructus Kochiae) can dispel wind and relieve itching.In addition,according to the actual situation of syndrome differentiation,Professor Xu choose the treatments of purgation,harmonizing,clearing heat,warming the interior,tonifying,regulating qi,regulating blood,expelling wind,moistening dryness,removing dampness and phlegm,digestion,insect repellent and other evidence-based treatment for tracing the origin and source.
