乳腺原发癌和淋巴结转移癌中TGF-β1和p53的间质表达及临床意义 被引量:3

Stromal expression of TGF-β1 and p53 in primary breast cancer and lymph node metastatic cancer and their clinical significance

摘要 目的探讨乳腺原发癌和淋巴结转移癌间质中TGF-β1和p53的表达及预后价值。方法收集有淋巴结转移的非特殊型浸润性乳腺癌160例,采用免疫组化EnVision法检测TGF-β1和p53在乳腺原发癌及相应淋巴结转移癌间质中的表达,并结合临床病理特征及预后进行相关性分析。结果在乳腺原发癌和相应淋巴结转移癌之间,TGF-β1和p53的间质表达一致率均为87.5%(140/160),两者的表达呈正相关(r=0.603,r=0.604,P均<0.001)。生存分析显示,在乳腺原发癌和相应淋巴结转移癌间质中,p53阳性患者的生存率低于p53阴性患者(χ^(2)=7.163,χ^(2)=6.894,P均<0.05);而TGF-β1表达与患者总生存率无关(χ^(2)=0.181,χ^(2)=1.065,P均>0.05);TGF-β1和p53均阳性者的总生存率低于TGF-β1阳性和p53阴性者(χ^(2)=5.616,χ^(2)=4.796,P均<0.05)。Cox单因素分析结果显示:肿块直径、转移淋巴结个数、ER、PR、HER-2的表达及乳腺原发癌和相应淋巴结转移癌间质中p53的表达,均是患者预后的危险因素(P均<0.05)。Cox多因素分析结果显示:转移淋巴结个数、HER-2的表达和原发癌间质p53的表达,可作为患者预后的独立危险因素(P均<0.05)。结论在乳腺原发癌和相应淋巴结转移癌之间,TGF-β1和p53的间质表达均呈明显正相关;p53的间质表达提示预后不良,间质TGF-β1和p53均阳性患者预后差。 Purpose To explore the expression of TGF-β1 and p53 in the tumor stroma of primary breast cancer and corresponding lymph node metastatic cancer,and to analyze the prognostic value.Methods A total of 160 cases of breast invasive carcinoma of no specific type with lymph node metastasis were selected.Immunohistochemistry of EnVision method was used to detect the expression of TGF-β1 and p53 in the tumor stroma of primary breast cancer and corresponding lymph node metastatic cancer.Their relationships with clinicopathological parameters and prognosis were statistically analyzed.Results Between primary breast cancer and corresponding lymph node metastatic cancer,the stromal co-expression rates of TGF-β1 and p53 were 87.5%(140/160),and were positively correlated(r=0.603,r=0.604,P<0.001,respectively).Survival analysis showed that in the tumor stroma of primary breast cancer and corresponding lymph node metastatic cancer,the survival rate of patients with positive p53 expression was lower than that of patients with negative p53 expression(χ^(2)=7.163,χ^(2)=6.894,P<0.05,respectively).The positive expression of TGF-β1 was not associated with the overall survival rate of patients(χ^(2)=0.181,χ^(2)=1.065,P>0.05,respectively).The overall survival rate of patients with positive expression of TGF-β1 and p53 was lower than that of patients with positive expression of TGF-β1 and negative expression of p53(χ^(2)=5.616,χ^(2)=4.796,P<0.05,respectively).Cox univariate analysis showed that the tumor size,the number of metastatic lymph nodes,the expression of ER,PR and HER-2,the expression of p53 in the tumor stroma of primary breast cancer and corresponding lymph node metastatic cancer were all risk factors for the prognosis of patients(P<0.05).Cox multivariate analysis showed that the number of metastatic lymph nodes,the expression of HER-2 and the expression of p53 in the tumor stroma of primary cancer were independent risk factors(P<0.05).Conclusion Between primary breast cancer and corresponding lymph node metastatic cancer,the stromal expression of TGF-β1 and p53 is positively correlated.The stromal expression of p53 suggests poor prognosis,and the positive expression of TGF-β1 and p53 in the tumor stroma can predict worse prognosis.
